Brief Answer:
PCOD

Detailed Answer:
Hi
Greetings
From your scan report I presume that you are having polycystic ovarian desease. That may be the reason you are not getting periods regularly.Ovulation may not be happening because of multiple peripheral follicles.
Another problem of PCOS is increase in weight. You have not mentioned your age and marital status.
I suggest you to consult a gynaecologist snd get yourself evaluated.
Proper medicines along with regular exercises will regularise your periods.
Hope you are convinced. Regards
