What Does My TVS Scan Test Report Indicate?

Question: Hi I am suffering from bilateral polycystic ovaries ,am under medicines like metaformin,b-long..I had ubiphene tab 50mg but in my follicular study none of the follicles turned out to be dominant hence doctor advised to wait till next cycle and asked me to get my TVs scanning done. .

todays tvs scan report shows -
Q
Urinary bladder is well filled distended with normal wall thickness. Lumen is clear.no calculi/ mass lesion noted.No diverticulum noted.

Uterus is anteverted,normal in size,measures 6.7x3.7x4.1 cm and shows normal outline and myometrial echopattern.No focal SOL noted.

Bilateral ovaries appear bulky and show multiple small follicles (at least 20 in number and each measuring 4-7mm diameter).

Right ovary volume 9-10.0cc
Left ovary volume 10.5cc

No adnexal mass lesion seen.
No free fluid in POD.

Impression: bilateral Polycystic ovaries.

I wil be starting to have ubiphene 50 twice from today dis time it's 100mg dose...what is my reports saying i know I have cyst issues will I b pregnant m 31now and m weight is 63 and am asked to get this down to 57 naturally..

Is your husband's semen analysis normal ?
Is your tubal testing done in the form of HSG or laparoscopy ?
Since how long are you trying to conceive ?
Yes, weight loss, Metformin etc will help in control of polycystic ovarian syndrome.
There is good chance of ovulation and conception if your partner's semen test is normal and your tubes are patent.

Hi s it was my first follicular study when the ovulation didn't happen.
Semen test report is attached fya.
Hsg was also done BT the doctor accidentally didn't mention wht is actually in the scanning so not sharing the report kindly check the scan and let me know ur opinion it was done on 30th may 2015.

U said no cyst but then why is the impression given as bilateral Polycystic ovaries .

Sorry for the delay at my end, the website was just not responding.
The semen analysis test is normal.
Regarding the HSG, it is essential to have the report.
The images are not clear, and only the doctor who performed the study, can give the exact interpretation.
In my opinion, the right tube is patent, but I am not sure about the left tube, since the image is not clear on that side.
Regarding polycystic ovaries, that term means that the eggs in the ovaries turn into small cyst like follicles, leading to irregular cycles and irregular ovulation, thus leading to difficulties in conception.
An actual cyst in the ovary is different from the term polycystic ovaries which is more of a hormonal imbalance.
With higher doses of Ubiphene, there are good chances of conception this time.
Please let me know as the follicular monitoring progresses, what are the findings this time.
If you ovulate well, then chances of conception are good, as the semen analysis is good, and one tube is definitely open
Try for 3 - 6 cycles of follicular monitoring, and then 3 - 6 cycles of IUI ( intra uterine insemination ) and if all of that fails, you can go for a laparoscopy to assess patency of the fallopian tubes.
That is my plan of treatment suggested for you.

What is IUI.

Well gyni here told me today the follicular study on the 6th day and see if their is any improvement seen if not then she asked us to wait till next cycle to increase the Ubiphene to 150 mg and see if their is any possitive progress .

One last question if i ovulate well shud i discontinue the tablets to avoid issues like miscarriage or something ..is it really ok to continue tablets will that not impact the conception ...Have seen my friends faced the loss due to miscarrige and i really dont want to go through any precaution that i need to take care when we try to mate.

And also can you help me know how to ensure the discharge is complete going inside and not spilling out.
And post trying is their anything that i need to take care or avoid doing skipping exercise or yoga where the abdominal movements are high,travelling anything that i need to avoid to increase the chance of pregnancy.Does the excitement impact conception.

I am eagerly waiting to be a mother and i just dont want that 1% chance to loose it ...

When the follicle ( egg ) is ripe, injection is given to induce ovulation ( trigger rupture of egg ).
Then your husband's semen is processed in a lab to make it of best possible quality and it is injected inside your cervix with a syringe.
It maximizes chances of pregnancy.
Which tablets are you talking about discontinuing ?
Please take folic acid ( Folvite ) 5 mg per day tablet daily to reduce chances of miscarriage.
Avoid stress, travelling after intercourse.
After having sex, hug your knees to your chest and lie down on the bed for 10 minutes, after that use the washroom.
You can continue routine exercises.

Today on my 10th day i did do the scanning and doctor found 4 large folliciles and one which is of 15 being dominant.

doctor said ideally when its 13 is when i had to give injection so we cant wait for long and so get the 2nd scaning done on 8th.

Can you check my report and tell me what is in my report and what injection is she talking about.what is the issue if you have more than one dominant foliciles.

Is the pregenancy in risk and post injection will all them rapture or is it possible that i avoid injection am scared of injection dont know where is she going to give me and how painful is it gona be.

I have gone through the reports attached by you, and I cannot find the report of scanning of the 10th day.
Please review and attach the report, so that I can go through it.
When the egg size is around 18 mm atleast, then injection is given to induce rupture of egg, ovulation.
SO once the follicle grows more and reaches optimum size, that injection is given to trigger ovulation.
If you have more than one dominant follicles, and more than one of them ruptures, then possibility of twin pregnancy is there.
There is no need to be scared of injection.
It is not painful at all, is given in the buttocks or in the arms.
There is no need to avoid injection, as it will help you to ovulate properly.
The pregnancy will not be at risk.
All may not rupture, only the largest one or two will rupture.
Please dont worry.
Send the report so that I can review it.

am still worried on the injection,will the injection end up in successful pregnancy.
i have seen twins born with leg or head or both attached is that because of multiple dominant follicle.

ET of 6 mm is good enough for day 10 of the cycle.
The injection just improves chances of rupture of the egg follicle that is mature, it is not a guarantee for pregnancy.
Twins are born when two egg follicles get fertilized or when one egg is fertilized with 2 sperms.
The abnormalities that you have seen , generally happen when twins are born out of one egg fertilized with 2 sperms.
When twins are born out of 2 separate dominant follicles, such abnormalities are not seen.
ANyway, yet there is no guarantee that 2 follicles will be fertilized in this cycle.
So do not worry about that.

Have uploaded my two reports.

Doc dint give me any injection as their is 4 good follicles which was equally growing and she was scared if the side effect increases I may need medical help hence she said she wil monitor and check if anything is rupturing by itself if not den she wil rduce d dose and give the injection in my next cycle.

What shall I do I have asked m husband DT we wil try from today if sumthing happens den it's good but when radiologist said it's real good egg on my of them shud idealy rapture right...

Well, yes, all of them are good eggs.
Before deciding about injection trigger to rupture them, I think you should go for further follicular monitoring.
ANd yes, if all 4 are growing well, then you should not take the injection.
There is risk of overstimulation if all 4 rupture.
Naturally , without injection either, atleast one of them should rupture.
So you can definitely try naturally and see what happens.
Also , in the next cycle, dose can be reduced, to reduce risk of overstimulation.
And you should continue the follicular monitoring to confirm that the follicles have ruptured naturally on their own.

My doctor dint recommend me further scanning so the last scan report that you saw on 8th apr remains the last report.

On 11th when i had appoint was my 15th day she said if pregnancy happens naturally it is good but i will not be changing the medicine or treatment for next month if you are not pregnant.

That day we had sex and i was lying i started getting cramps a pain which remained for hours not sure what was that.I took it as ovulaiton pain reading articles in net and we tried the next day but sperms just spilled outside accidentally and not sure if anything would have entered post that no sex till this date.

My doctor is asked me to have duphaston till i do my pregnancy test incase i miss my periods this month.

My period date is 28th apr if i assume its going to be on time,i did a home pregnancy test yesterday it was negative,i see white discharge past 2 days does this mean am ovulating again.
i try my level best to not think anything about it at all but am unable to do it as well.

What should i do XXXXXXX

Well, the doctor should have continued monitoring till ovulation was confirmed.
There was no reason to stop the cycle without documenting ovulation.
And yes, white discharge does not necessarily mean you are ovulating again
It might mean an approaching period, early pregnancy or nothing at all
Just wait until your next period, and start a new cycle, but please confirm ovulation.
Even now, you can get one scan done to check if the 3 mature follicles have ruptured or not.

The doctor was clear that their is not going to be any change in the treatment for the next month and she said if we want we can do another scanning to check the progress or ovulation when i showed interest to know what will finally happen after doing so many scannings.But next time will surely be clear that we want to document to check ovulation.

i appreciate your advice and will suerly get a scanning done htis wed to check the status of 3 mature folliciles.One question what i have is if the ovulation is happend of all the 3 say on 11 apr will the scanning still triger out if the ovulation happend.

Please get the scan done even now, what cycle day is it right now ?
You cannot know how many follicles ruptured, that you would have known only if you had got the scanning done sequentially.
You can travel slowly on a bike, avoiding heavy bumps and jerks.
Eat healthy , avoid eating outside.
Avoid stress or exertion of any manner.
Do not take any medications for cold, cough, fever etc without consulting your doctor first.

Today took a home pregnancy test and saw two lines one clear another not that clear a light pink line now is this due to duphston tab DT m having or is their a really good news.should I carry d test result to doctor today.

How do I read so not DT clear line as.have attached the image FYA.

Hey she said it's early pregnancy and so said wait for a week and then do another hpt and check if it's positive again..also said now der is no point in scanning as post period due date ie 28th it's too early to detect ..she àsker to continue duphston myocyst-m but she said b-long I need to discontinue and have folic acid as b-long doesnot contain folic acid m nt sure as to why then she asked m to have dem till this date coz I remember she told it's folic acid n now she finds out its not or may b I wud have overheard..
Is their any issue if the pregnancy test shows positive this early as per her it's all thru medicines that's happend so risk of the location is der..so she also asked me if I get any sort of pain below the abdornmal to immediately get the CT scan done....but am confused is she trying to tell me the pain if come is good or bad..or does this mean DT the pregnancy is happening outside the cell...

I have switched to folic acid.
Today i had cough so when i was sneezing i cud see kitle white discharge dts fine right and till now have no pain felt nor any vomiting sensation felt.
Until its confirned should i b consius d way i sleep or roll.. Well i dont have dt habit bt i do sleep ok one side in between m sleep is dt ok..

Now am not getting any cramp's these it looks so normal for me bt food eating has increased and the gastric n breast pain when i wake up stil remains same.

No i havent done ultra sound scan yet... Since u said to wait for a week n get the hpt done and den after 15 days to get the ultra sound done so i din go yet..
My period due date was 28th apr so we counted from 29th till date will be 8days... Is iy ok if i do on sunday (8th may), that is on 10th day... And on 14th may ultra sound done..
Not sure if am calculating right...

And hey is their anything that i can do to make sure der is breast milk.. I see few friends of mine who fail to breast feed not sure why their is no milk but also see when they gave birth to the next one they did have milk coming.

Please do not worry about breast feeding right now, I am very sure that when you give birth, by that time, the breasts will be prepared and you will be able to feed properly.

What does the count describe about the pregnancy, is it healthy pregnancy, how many weeks, twins or single.

I am continuing to have duphaston and myosis-m and folic acid tablets.

Today morning found again a litle darker brown discharge i kept some cotton n den washed off felt a little scratching sensation now i dont feel anything.
For a sec thought its bleeding it was a litle more and i also felt a litle back pain or some uneasyness just like when i used to have at the time of my menstrual cycle.

Please get the ultrasound scan earlier then, maybe even tomorrow.
The discharge and pain is not a good sign.
Have you ever been pregnant earlier ?
The count only signifies pregnancy, it does not show a healthy pregnancy, or weeks, or twins or single.
Only ultrasound scan can provide that information.
You should continue folic acid tablets but stop the other two.
I would suggest vaginal Susten progesterone tablets ( to be inserted vaginally daily one tablet at night ).
However, please get the scan done and then discuss this with your doctor.
Let your doctor take the final call on this.
Please let me know the scan result tomorrow.
