Question: sir,
i am XXXXXXX from XXXXXXX XXXXXXX my mother aged 64years underwent MRI Scan recently and diagnosed with accute infracts in left medial temporal lobe, occipital lobe, posterolateral thalamus, adjacent posterior corpus collasum. Multifocal lacunar infracts in left cerebellar hemispher, pons, bilateral thalami, ganglio-capsular, pericentricular, corona radiata and centrum semiovale regions. punctate micro haemorrhages in left cerebellar hemisphere, pons and bilateral thalami regions. please suggest the medicines for this and also explain what this it indicates

1. Acute infarcts in left medial temporal lobe, occipital lobe, posterolateral thalamus, adjacent posterior corpus collasum.

This explains a state where there is sudden stop of blood supply to certain important areas of the brain. This can cause various levels of neurological symptoms including paralysis and inability to speak, decreased vision and cognitive abilities. This has happened within the last few hours or days.

2. Multifocal lacunar infarcts in left cerebellar hemisphere, pons, bilateral thalami, ganglio-capsular, pericentricular, corona radiata and centrum semiovale regions.

These are old small areas of absent blood supply on certain areas of brain. These might have occurred over months or years and are more like a slow process of deficient blood supply. This can cause slowly deteriorating neurological and higher mental functions.

3. Punctate micro haemorrhages in left cerebellar hemisphere, pons and bilateral thalami regions.

This is a feature seen in chronic hypertensive encephalopathy commonly and is small areas of bleed into various parts of brain due to persistent high blood pressure effects.

With the above findings, patient is not in a serious condition and is stable. She may require supportive care for sometime and should be given medications as prescribed by the neurologist.

Regarding medications prescribed
Exact name Rabiva is not traceable but Rabix (Rabeprazole) is given for gastritis and taken before food 1 - 0 - 0.

Noofit is piracetam and works on the brain by protecting it from low oxygen states.

Prexaron is citicoline and this also acts on the brain and has psychostimulant action.

Prescribing the medications is justified and they will help regulate the blood flow and prevent further damage to the brain. The neurological symptoms will take time to improve and you should follow up with the neurologist regularly.

thanks for your valuable suggestions. sir she is having severe head ache on the areas of the clot. should we take any medicine like dart to overcome pain or it is included in the above medications? the generic name of OASCLO is Aspirin 150mg and clopidogrel bisulphate 75mg.and Fenovastatin ( Atorvastatin calcium 20mg)

Thanks for writing back with an update.

The medications OASCLO (Aspirin 150mg and clopidogrel bisulphate 75mg) is for prevention of blood clotting and Fenovastatin (Atorvastatin calcium 20mg) is to reduce cholesterol. These medications are right for her and please continue under your doctors supervision.

Since she is taking aspirin, there is no need to take DART which is a combination of pain killers. The headache will be taken care of by OASCLO in most cases.

She should not be given DART because it contains propyphenazone which can interact with clopidogrel.

In severe headache you can give her one 500 mg tablet of crocin (paracetamol) after food on emergency basis. For continuous severe headache she must consult doctor again.
