What Does My HIV Test Reports Indicate?

Question: Hi Doctor,

I had below test results at 37 and 87 days
Hiv 1-2 antibody - negative
Hep b and c - negativ
Vdrl - neg
Hsv 1-2 - neg
Ana 1:100
C protein - neg
Esr - neg
Ecg - normal
Tmt -normal
Lft- normal
Uric acid - 490
Tsh - 5.5
Vitamin D -17
Hiv pcr rna quantitativ - <72 ul/ml ( after 87 days, assay senstivity start from 72)
Even with all such test , my fear of hiv and std are not setteled (with negative hiv test and 3 months) , today at 5 month mark i am dealing with severe shortness of breath , heart palpitaions , on and off pain in multiple body parts( legs , arms , neck ,back etc), sweating(under arms).
Please help me on my status, do i need to retest after 5months. Is my results conclusive or going to change after 6 months.
Why CDC quoting guidelines sometime 3 months and sometime 6 months. What is correct for normal healty persons.

As on 37 and 87 days after exposure HIV 1 & 2 antibody, HIV RNA PCR, STD panel and other routine blood tests are negative then you are absolutely free from HIV and other STDs like syphilis, herpes, hepatitis so no need to worry.

Your uric acid level is bit on the higher side but not troublesome. Your symptoms are due to anxiety or stress only as per my opinion. For further evaluation and advice, consult your local physician and follow the treatment regimen. I recommend regular exercises or yoga to keep your self relaxed and to alleviate anxiety or stress. hope this was helpfull. For any further query, write to me again.

Dear Doctor,

I had all encounter as protected sex on 2-6 sept 2015 and couple of unprotected bj and french kiss to csw's.(i was too drunk ) so not able to check the integrity of condoms post intercourse.
Had no symptoms appeared so far except all such lingring issues from last 5 months.
- body pains
- feeling of lymph nodes( neck, groin, back etc)
- mouth ulcers( 1-2 weeks back only)
- weak vision/senstivity to light
- extream weekness (drowsiness,feeling of fall down)
- extream breathing sensations (very much concerned of this symptom( does it menifestation of early hiv infection))
- redness on skin ( going and coming )
- sometimes ( feeling something stuck inside throat
Now i have few questions:
1- first i had negative antibody results on 87 days and than i got my hiv pcr rna report after 2 days (89 day). Why in report results mentioned as <72 ul/ml not as"negative". In interpretation its written has "hiv rna detected <72 ul/ml. It confusing me alot that at 3 months might be antibodies are not developed but why copies are so less . Does both test eliminate posibility of infection at all and considered as 100% ( scientifically and medically).
2- All test are done at XXXXXXX hospital through vitro eci kit. Hope all holds good in XXXXXXX
3- first i thought 3 months are good and conclusive XXXXXXX and negative test at 3 months will settle my symptoms and anxiety.But its not happened so far. Now at 5 months my SOB and weakness symptoms are more severe.if m clinically fit how to get rid of such symptoms , or i am so unlucky that might be my status change after 3 months.
4- can i go and start unprotected sex with my wife.i had lot of guilt from last 5 months and leared my lesson from this mistakes bigtime.
5-No other such activities perfomed in last 5 months.

Pls help with ur guidennce and inputs.)

The negative antibody test at 87 days and negative RNA PCR at 89 days after infection definitely eliminate the possibility of HIV so no need to worry at all. These tests are 100% reliable and conclusive after 3 months. The cut of value for RNA PCR is < 72 copies per ml. Any value below 72 copies is considered as negative so don't bother for this and allay the fear of HIV.
XXXXXXX hospital is reputed institute so the results were reliable and there is no such possibility of status being changed after 3 months. Also there is no need to go for any further test for HIV. Your symptoms are not related to HIV. They can be due to stress or anxiety or any other medical condition.

You can definitely start unprotected sex with your wife as you are risk free. As your friend I will advice to stay away from such unsafe sexual practices to avoid future risk of HIV and other STDs. Thanks for the vote of confidence and amicable suggestion and guidence.

I am so much worried of this breathing discomfort along with weakness in body.

Sometimes i feel my hands and feet are burning mild and close to severe.
Burning mouth and tonge.


My worries are in the same forum of healthcare magic couple of ID doctors are suggesting 6 months test of patients.
In multiple sites like body.com , medhelp , forumpoz etc and doctors like dr XXXXXXX and dr.hhh and my internal medicine doctors in XXXXXXX appolo etc are quoting my 3 month test are sufficient and concluded.

But my fear is if concluded than why still such symptoms. Doctors i have two innocent lifes are also staying with me ( my kid and wife) and i dont want to become trouble for them. Hence looking for upmost scientific and medical assaurence to start similiar life with them before such episode started 5 months back.

Pls suggest doctor .... Does above points will change ur opinion or should i go unprotected with my family with no addtional fears.

And secondly ... Can anxiety and stress create such severe and mild issues of SOB , pain and weakness

The test result after 3 months of exposure is 100% reliable and conclusive so your internal medicine doctor is absolutely right regarding this. You can take his expert opinion for breathing problem or consult some chest physician for the same.

The burning in hands and feet along and weakness can be due to vitamin deficiency so get some tests done to rule out the cause and take multivitamin tablets like neurobian-forte or supradyn etc.

Excessive stress or anxiety will surely affect your health so try to avoid it as much as you can. You are free from HIV so nothing to be feared about. Be relaxed and enjoy your life happily with family.

Hi Doc,

This is my last followup question on this subject.please help with ur suggestion on below points:
1- So far i have consulted 5 (internal medicine doctors from XXXXXXX Appollo, fortis , Vedanta and another reputed hospital) along with you and Dr.Basti) all have recommended me i am free from hiv disease and not to worry at all.
2- Problem with me I believe is not hiv but fear of hiv , guilt and security of my loving family. If my by results m medically and clinically is good than help me how to get rid of these ongoing symptoms.
3- I have tried anti depressents like xanas , cipralex but it didnt suited me at all and created more troubles. Pls help me with alternate way.
4- Mind is a very power thing , it almost concluded me hiv positive 2 months after start of this episode even with negative antibody test at 37 days. Then again i have tested neg at 87 days. But still its playing tricks with me and not letting me to live peaceful life due to these symptoms. I am trying hard to convence it but some or other how it wins. How to stabalise the positive thoughts and to forgot this hiv episode.
4- I am doing excecise daily 1 hour on ( 30 mins trademill, 30 mins cardio) to settle my anxiety and sob( if related to it) from last 1 week.
5- I have checked my vit b12 and Vit d on XXXXXXX 2015 and the value was 32 and 5.5. Than i have started my meds neurobin forte and vit d tabs from 3 months and re tested on sept 2015 with b12 863 and D 33.
Now again tested for vit D on XXXXXXX 16 with value of 17. Should i retest for vitamin B12 again. Since from last few days i am experiencing mind heat on plam of my foot.
6-i have family problem of low nd high tsh from last 10-15 years and with me almost 8 years and daily i am taking one pillof tyroximine 100 mg. pls suggest vit d , b12 and tsh deficiency are not considered as auto immune deficeincy and no impact on hiv test and results.Since i have heard some autoimmune def causing delay on hiv results like hep c , cancer etc.
7- currently i am taking below medicines
Tyroximine 100 mg - 1tab/daily
Becasulez - daily
Vit D 500000 IU- once in a week.
Should i start any other vitamin medicines.

Pls suggest.

You are absolutely right that it is the fear of HIV not infection which is causing all the troubles nothing else. The test result after 3 months is definitely reliable and conclusive so no chance of any unexpected result. You are free from HIV.

No need to take anti-depressants because these medicines have many of their own side effects. Continue taking medicines for vitamin deficiency and it has no impact at all on the result of HIV test. Do exercise, yoga and meditation for the piece of your mind. Forget about the past and live happily with your family.

You can consult physician for the management of your symptoms. If not of help then some psychiatrist or counselor will help you to get out from this. You can get back to me for any further clarification if needed at any time.

Last and final thing from my side.Since i heard same thing "hiv negative" from multiple doctors(almost5-6). So i am beliving hard on that part.
One last questing which need your help to settle my fears are-
1- Why i am getting hits of intermittent breathing discomfort along with weakness and feeling faint for short period of time.Is it entirely mind game(anxiety) and how to settle the same? Or could be menifestation of medical problem.Trust me before 5 month i was a very heathy guy with good immune system which i can proud off( not even getting flu, loose motion , fever etc once in a year , even eating junk food , smoke and drink habits).
2- I have consulted multiple physician / internal medicine documents and they are also reffering the same to calm down so if m out of danger. Have u ever seen case similar like me and how to get the fear of if/else out of mind.

3- After the encounter in sept , i had quit smoking , drinking and all other bad habits since due to drink i was not able to took the right decision and get into this first and last time. Does quit smoking had any withdrawal impact now after 5 months?
5- last and most critical important point, i love my kid like anything and my family.Please with all respect i would like to ask , i am good to like without any fear with my family and trusting on the jugement whatever symptoms and physical problems are happening with me is nothing related to this disease and its all psycological fear which i have to address now at last toll gate of this entire 5 month suffering and punishment as a return of my mistake

It is just the anxiety or the fear of HIV which makes you feel paranoid nothing else because you are absolutely risk free and safe so forget the past, avoid thinking too much for HIV and live normal happy life with your beloved family.

Consult some psychiatrist if not able to cope up with stress or tension. Continue to take all the medicines that you are taking except for anti-depressants. Take healthy and nutritious diet to boost up your immune power. Do yoga and regular exercises for your mental as well as physical relaxation.

Again I will say there is nothing to be worried about in your case. You will be fine and healthy as before, just what you have to do is throw away all bed ideas and thoughts from your mind and not to take unnecessary tension. Hi Doctor,

One more point , i have uploaded couple of pics for ur persual.

I am not getting why such redness spots coming to my body and going after 4-5 days.
Some red dots also appearing in multiple body locations. Is there any corelation with my fears or its just normal rashes or redness due to climate change or something else.

The rashes and redness is mostly due to what you had said climate change or allergy but not related to HIV. Take some anti allergic medicines like Avil or Cetrizine and consult dermatologist if needed for further management. Dont worry it will be fine.
