What Does Calcified Granuloma In Right Lung Indicate?

default
Posted on Wed, 30 Jul 2014
Question: Hi. I developed a sensation of mild breathlessness, not shortness of breath, just a feeling like i cannot get enough air in my lungs, started about 2 weeks ago. I yawn and take deep breaths more frequently. Also, i have had constant pain in the right clavical area for about 3 months, that actually feels like a strained muscle, except it doesn't go away, i just assumed it is arthritis and took motrin & asperin. This pain increases for a couple days after doing upper body excercises at the gym.
I'm 58 years old, female, 227lbs, cholecystectomy for stone about 4 years ago. Elevated triglycerides (333) and cholesterol (217). Have been taking simvastatin 40mg daily for years. AST(SGOT) 58, alk phos 136, total ALP 125. O2 sat is 98%. BP 130/70.
Total thyroidectomy 6 years ago as a result of a positive needle biopsy, but pathology negative for cancer.
Ive been unsuccessful to lose weight in spite of my efforts. In fact, I've gained 27lbs since Sept 2013.
My abdomen seems to be increasing in size, I feel bloated and my abd is larger above my naval.
Doctor d/c'd the simvistatin, ordered tri clor instead. (?spelling)

CXR: 2 views.
No focal infiltrate or effusion. Calcified granuloma right lung similar to prior (2011). Density probable reflecting appearing eventration of the posterior hemidiaphragm is now noted on the lateral, 3 month followup exam advised.

A CAT scan and ultrasound of liver has been ordered.

What exactly does the CXR reading mean in layman terms?
Is it ok to continue excercises at the gym?
What can I expect in my future?

Brief Answer:
It mean healed infection.

Detailed Answer:
Hi
Thanks for using healthcare magic. I, Dr Abhishek Kapoor, would help you in this query.

Let me first explain about the granuloma. Granuloma is a inflammation found in many diseases. When any infection happens, then body defence mechanism works and it leads to collection of few cell at site of inflammation. Collection of these cells is called granuloma. With time, when it heals, calcium deposition happens and this leads to calcification.

May be in past, you may have any lung infection and when that healed, it would have caused to calcium deposition. Calcified granuloma is indication of complete healing of past infection and in x-ray, you can see it in white patch. In your case, during x-ray, it is also showing some reflection on part of diaphragm. There is nothing to worry about it.

Weight gain could be due to hypothyroidism and it could also be the cause of anxiety symptoms in form of breathlessness or chest pain. In that case, get your thyroid profile done.

Pain at area of right clavicle could be due to tendinitis. It could possible due to gym or lifting heavy weight. Better to consult a orthopaedician for proper management. For few days, you can avoid gym. Mean time, you can take pain killer on sos basis.

If you have more query, you can ask.
