Question: extreme back pain.. muscle right of the waist line. Heating pad used too long? Did a lot of indoor door removal from hinges. Ice packs at10 minute intervals do help. Ibuprofen no longer helps. Male in great shape. Works out daily till now. Pain 4 days running. NO PAIN UNTIL ATTEMPTS TO MOVE OUT OF A SEATING POSITION. Can walk after getting up.

Pain which is triggered by position changes or movement is suggestive of Muscle strain/pulled muscle.

Try avoiding using heat pads until you notice the pain has decreased. Until then use ice packs only on the area of pain. Once the acute severe pain has decreased, you can switch to heat therapy.
You may take Advil 400 mg four times daily to help with pain.

Take adequate rest. Avoid exercising or lifting heavy weights until the pain subsides.
Sit on a comfortable chair with adequate back support.
Sleep on a comfortable bed.

The pain should subside in next 6-7 days.
