Question: Hi, I tested positive in a blood test for pregnancy three days ago and was told the number was 1065. As we did IVF, I would like to know if Progynova, which I am taking three times a day, plus progesterone pessaries (400ml) morning and evening, affect this number or does the number relate to something else entirely? I read somewhere that a first pregnancy test done at the time I did mine needs only to be above 90. Therefore, could 1065 mean I might be carrying multiples or is it affected by the medications I'm taking? I have another blood test next week when I'll be 5 weeks and 5 days pregnant. Ideally, what would my number be then?

Brief Answer: Hello, I would be happy to help you.... Detailed Answer: Based on what you are telling me, this HCG level was drawn at about 4 weeks gestation. This level is perhaps higher than I might expect at this gestational age. Knowing that IVF with implantation of more than one embryo is highly associated with multiple pregnancies, then I would say that the possibility of twins is very high. Also, the medications that you are on will NOT affect the HCG levels. HCG is a hormone produced by the pregnancy itself and is not affected by the medications that you listed. Thank you for your reply. When would you expect I could confirm whether I am pregnant with multiples or not? I have a scan booked at the seven-week mark - on 14th March - and another blood test next Wednesday and the Wednesday after, in the meantime. Would you normally expect to see how many embryos are developing during the seven-week scan or at a later point? Thank you again.

Brief Answer: Thank you for the followup... Detailed Answer: The earliest that you can see a gestational sac in the uterus is at an HCG level of 1500-2000. This number is probably going to be a little bit higher in a pregnancy where there are multiple sacs. In all likelihood, you would be well above 2000 by now. An ultrasound at this point in time would probably show if you had multiples, but it would be too early to see fetal heart beats. I would agree with waiting to do the scan until you could gain the most information unless a problem developed in the interim. I hope that this helps!
