What Does A Growing Painful Red Patch Near The Toes Indicate?

Question: I had a small red patch in the inner side of my right big toe two years ago. With time the red patch has spread in the inner toe area. Now when I walk, the big toe is extremely painful. I had an accident two years ago before this red patch appeared, which hurt my right ankle by tearing some ligaments and there is a slight growth of scar tissue there. I don't know whether there is a connection. I am 29 years old , male, height is 5ft 6 inches. I am both physically and mentally troubled by this as the pain is increasing and the red patch is spreading. In the area of the red patch, there is a numbness when compared to the other areas. When my leg is highly active (eg: doing exercise) the pain kind goes away (I don't feel the pain). When my leg is at rest for like 2 hours, and then when I start to walk again, it is extremely painful for the first couple of minutes. Then the pain gets decreased. The pain is concentrated in between the metatarsalia and the phalanges proximales. Doctors have checked for my blood for possibility of gout and told me that the uric acid levels are completely normal and the full blood count is normal as well. I have attached a picture of the red patch. This is EXTREMELY painful. I feel really scared as to whether I am going to lose my finger due to some condition. The skin color in that red area, turns blue in the morning and when doing very little activity with my leg and also when in very cold conditions.

I can assure you that there is no chance of loosing your toe. I would recommend you to get MRI to confirm the diagnosis. Also for pain relief I would suggest you to take Brufen. Hi Dr XXXXXXX If it is due to vascular malformations, would that be a serious condition? What would you propose in that kind of a situation? Will my leg have to be operated? (like surgery) Thank you

Brief Answer: usually not harmful Detailed Answer: Hello, Vascular malformations are usually not harmful. So there is no need for removal in all cases. If it is painful, then there are injection available to make them dry. These are all called scleroscent. Also embolization is used to block feeding vessel. If these techniques fail then last option is surgery. I would suggest you to meet with your surgeon to decide further management. Thanks for this followup question.
