Brief Answer:
Inflammed. Needs antibiotic and supportive measure

Detailed Answer:
Hi,

I went through the query and understood your concern.

1. Brown spider bite:
Usually brown spider bite go unnoticed as there wont be any pain or bite site may be mildly red. Most commonly, the bite site will become firm and heal over the next few days or weeks.
Very rarely, the local reaction will be more severe leading to blistering (common), necrosis (death) of skin and subcutaneous fat (less common), and necrotic lesions with deep wide borders (rare).

For now, since the swelling is increasing upto elbow with redness, this suggest inflammation with in the tissue. He needs antibiotic therapy along with supportive measures. He should take
1. Inj TT 0.5 cc if nort imunized.
2. Antibiotics: The selection of right antibiotic depends on your doctor's preference based on his local area experience for emperical therapy. For good tissue penetration of antibiotic I prefer floroquinolone group antibiotic along with metronidazole.
3. Continue hydrocodone 5 mg and Tylenol 625 Mg as prescribed by your doctor.

Better to get examined by your doctor and start treatment as explained.

I hope this information helps you. Feel free to ask for any further queries and I will be glad to answer them.
I wish him faster recovery and good health.

Regards
