What Does CMV Positive Torch Test During Pregnancy Indicate?

Question: My torch test report says both cmv igg (250) and igm ( 1.02 ) positive. I am 28 weeks pregnant. What are the risks to my baby.Please help.

Thanks for posting your query.

Your lab report suggests positive infection with Cytomegalovirus. As IgM can remain for 9-12 months after an acute infection, IgG avidity testing can be done to differentiate acute from past infection. Risk of fetal transmission is approximately around 30-40 percent.

You may be advised an amniocentesis to detect the presence of the virus along with an ultrasound. If no virus is detected and the sonogram is normal, watchfulness is maintained through serial sonograms and if needed, amniocentesis.

If virus is detected with a normal sonogram, injection of hyper immune globulin is suggested.

If virus is detected along with an abnormal ultrasound, termination of the pregnancy should be considered or injection of hyper-immune globulin is given.

As you also have positive IgG titres, it is more likely that you have a past infection.

At your stage of pregnancy, termination may not be advisable and as such, monitoring along with planned pregnancy care is given.

does high igg of greater than 250 means that i have had this infection in the past and it is not a primary infection and a re infection and therefore congenital risks are lower. or is it that since igm is also positive still there are potential risks

Presence of IgG means that you are already seropositive and had an exposure in the past; this in itself lessens the chances of the risk of congenital infection. Also, any exposure up to six months prior to conception carries little risk. IgM can be detected up to 9-12 months from the time of exposure to CMV and to be sure whether there is an acute infection, avidity test is a must. First time exposure during pregnancy, which is not there in your case, carries with it the highest risk.

Going by all these factors, there appears to be very little risk for your pregnancy.
Though you need to get monitored properly, you can rest assured.
