What Do These Routine Medical Check Up Reports Indicate?

Question: The routine medical check-up of my daughter reveals laboratory test result, which is by and large normal. The hospital doctor does not show any concern for it. But I have some questions, particularly its haematology part. Kindly let me know, if the high % Neutrophils & low % Lymphosytes indicate any kind of infection: Bacterial or Viral??

Neutrophils: 83%
Lymphocytes: 11%

Platelet Count: 151 thousands
BIOCHEMISTRY
KIDNEY FUNCTION TEST: All parameters Normal (Serum Creatinine: 1.2, which is only higher side of normal)
LIVER FUNCTION TEST: All parameters absolutely Normal (except Liver Serum: 198)
URINE EXAMINATION
PHYSICAL EXAMINATION: All parameters absolutely Normal
CHEMICAL EXAMINATION: All parameters absolutely Normal
MICROSCOPIC EXAMINATION:
Leukocytes: 2 – 4 /HPF
Others found Nil

I can understand your concern.
She has low total WBC count with low normal platelet count and high neutrophil count. It may be seen in viral infection. If she don't have any symptoms then no need to worry. Just repeat her complete blood count after a week.

If she has fever or other symptoms then she needs treatment.

If she has no symptoms then no need to worry. No any treatment is required right now. Just drink plenty of fluid orally.

Thank you for answering my question and your suggestion. I have instructed my daughter for oral intake (ORS & Fruit Juices) as much possible. But, I have still confusions in spite of below normal WBC Counts of my daughter. Kindly refer to the following medical interpretation on CBC test:

where, it is mentioned under the paragraph, 'why is it important?' - the first two points read as follows:

1. If the percentage of neutrophils shows an increase, it could indicate a bacterial infections.
2. Elevated levels of lymphocytes could indicate a viral infection.

My daughter's blood (cbc) reports: Neutrophils=83% & Lymphocytes=11%,
If we go by the logic of the above linked literature, we may infer it could be due to some Bacterial Infection as opposed to Viral Infection. It is also true that wbc count (=3300) suggests some viral infection.

Does it mean that she is having both Viral as well as Bacterial Infections??

Further, her lymphocytes count is very less (3300 x 11%=363), which indicates Lymphocytopenia. Do you doubt any Autoimmune Disorders or some other major problem and a cause of concern.

In fact, we live in a small town in the state of XXXXXXX which is lacking good medical facility & competent doctors. There is no Hematologist. Doctors are very arrogant. I am therefore very much dependent on internet literature. Being a non-medical professional I have difficulty in understanding the science of medicine.

So, I need your help. Please clear my above confusions and your suggestion so that we can solve any bigger health problem of my daughter in future.

It is true that neutrophils increased in bacterial infection and lymphocytes increased in viral infection. But it is not the rule and other parameters should also taken in to account.

In many viral infection neutrophils increased in early stage. Moreover your total count and platelets also favour viral etiology.


Please tell me about symptoms. If she has no any symptoms then no need to worry.
If you have any symptoms then please tell me the symptoms.

Dear Sir,
The question is therefore, whether we should further inquire into for any possible bacterial infections or not???
If you suggest that we should rule out bacterial infections in her case, then please name the tests that all should be done locally in the overall interest of my daughter's health issues that may be in a hidden state now.
I am also thinking of taking her to an advanced health care institute at Calcutta before end of this month.
