What Do These Lipid Profile, TMT Test And ECG Report Indicate?

Question: I m 28 years male....have mild hypertension , on concor AM 5, resting ECG borderline , LVH by voltage criteria, 2d echo with doppler normal. normal Lipid Profile ..i underwent TMT today, comments / results are resting ECG WNL. with stress no fresh ECG changes. Blunted Chronotropic response due to betablocker. No angina or arrhythmia. Normal haemodynamic response. Stress Test negative for reversible inducible ischemia...Plse interpret the results....should i go for further tests? is it now feasible to rule out now any heart disease / blockage? Regards..looking for the answer with great anticipation....

Hi

Your reports look fine and Normal. When were diagnosed to have blood pressure and how much was blood pressure than and now. Hope its normal. We don't make diagnosis of LVH by voltage criteria in individuals younger than 40 years of age and hence that is irrelevant for you. ECHO and treadmill are normal. Though not always but in your case it rules any blockages or any other forma of heart disease and currently you don't need any further investigations for heart.

I was first diagnosed with hypertension in 2009, during that time i had sweating with feeling of faintness...I visited a tertiary hospital he measures my b.p it was 140/90...then he advised me various investigations viz blood test, lipid profile, blood sugar, creatinine etc, USG abdomen, pancreas, renal arteries, spleen etc...all were normal..then he had my ecg...it showed lvh...however 2d echo was normal...then he prescribed me amoldipine tablet ...i had this tablet for atleast one year...then again i visited cardiologist in 2011 due to uneasy, shifting pain in chest in another hospital at Bombay...he again had my ecg..showed lvh by voltage criteria..he measured my b.p it was 140/90,,,he then had my 2d echo normal....in 2014, i had one more 2d echo with doppler, came out again normal...he prescribed me concor AM 5 , i m on the same medicine till then..Due to my anxiety feeling while visting a doctor, i bought a arm cuff home b.p monitor citizen company...i usually measure my b.p at home it comes normal..but when doctor measures it is always higher....from the last three months or so i m measuring the b.p at home,,,diastolic reading low usually low below 60 with pulse rate also low below 70 sometimes below 50...i play basketball in the evening... since i was feeling neck, shoulder and arm pain i agin visited the same cardiologist.. he had my ecg... same lvvh...he told me that it is not due to heart...he advised me xray spine..it showed mild spondylosis....since the doctor didn't recommended me further tests...i underwent the TMT for my re assurance, which as told by u is normal....i m having doubt that whether there might be some heart disease, which is not being detected....should i go for further tests...is it now clear that my heart is fine and i should go for another investigations related to other than heart...Plse guide..and what do u think it is due to...i have gastic, acidity problems also....could this be the cause....

You seem to be typical case of white coat hypertension. if your home monitored BP is normal or lower normal you need to have Ambulatory BP measurement done and if it also shows same there is room for reducing your medicine or may be stopping them all to gather over time. For your pain you don't have worry about the missed diagnosis and no need to do further investigation

Thanks doctor, for ambulatory b.p should i be hospitalised...or i can measure it being at home...i feel that i don't have high b.p since i measure it at home and it is usually normal,, even low sometime...whether the medicine i am taking i.e concor am 5 without hypertension can cause any problems to me...?

no it doesn't require hospitalisation. after connection you can go around with your regular activity. you will able to do everything normally except taking bath. XXXXXXX am is relatively safe medicine but has few side effect like leg swelling, low heart beat and fatigue.

can concor am 5 cause low pulse rate below 50....mine comes sometimes 48, 49 at home....is taking concor am 5 without having hypertension can cause any damage to my heart.....or can it cause some ill effects to my body?
I hadn't discontinued my medicine concor AM 5, during treadmill test, whether my test can be considered as conclusive / correct? or should i go again for the test with discontinue of beta blocker?

Yes it can cause low heart rate

yes it can cause low heart rate. but as you mentioned being an athlete also could be the cause. apart from causing low heart rate no other structural damage will be caused by taking concor.
Since you have attained required heart rate on treadmill it is definitive and informative. no need to repeat. we do that if you do not attain 85% of required heart rate.
