What Do These Fasting Blood Sugar And Serum Creatinine Levels Indicate?

Question: my fasting blood sugar is 132 mg/dl and serum creatinine is 1.5 mg/dl how to control it and what i do?

Hi,

Thanks for your question.

After going through your question, I can understand your concern.

Your Fasting blood sugar level of 132 denote diabetes which need treatment.
Also serum creatinine level of 1.5 (may be above normal level or may be high normal depending upon lab method) denote chronic kidney disease (CKD).

You have mentioned you are following exercise only and not on any medicines.

I would like to advise you to consult diabetologist for detailed clinical examination.

Following investigations helps in taking appropriate decision regarding management of diabetes and ruling out presence of kidney disease and if present appropriate management.
1) Check Fasting and post prandial blood sugar level.
2) HbA1c level.
3) Lipid Profile.
4) Repeat serum creatinine level to confirm the high value.
5) Serum urea, Blood Urea Nitrogen, Uric acid, Serum Electrolytes.
6) Serum Calcium, phosphorus.
7) Early morning spot urine albumin creatinine ratio.
8) Complete blood count.
9) Liver function test.
10) Urine examination.
11) Ultrasonography of abdomen and pelvis to know kidney size, shape, echotexture.
12) Fundoscopy to rule out presence of diabetic or hypertensive retinopathy.
13) ECG and 2DEcho of heart.

Depending upon all these investigations appropriate decision regarding antidiabetic medicines and measures to revert or prevent the progression of kidney disease can be taken.

Till that time keep following regular exercise, diabetic diet.

Hi,

After going through your reports most likely cause for your elevated creatinine level is multiple renal calculi, rather than diabetes.

You should consult nephrologist or endocrinologist for metabolic evaluation for the cause of multiple renal calculi and appropriate management for its prevention.

Regarding your diabetes (Fasting blood sugar of 132 confirms the diagnosis of diabetes) you can start Tab containing Metformin but it requires close monitoring of serum creatinine and eGFR (Estimated Glomerular Filtration Rate).
Other safer option for diabetes considering your kidney status are -
- Insulin.
- Oral Medicines like - Siagliptin, Linagliptin, Pioglitazone and few more.

Appropriate option can be considered depending upon your postprandial blood sugar and HbA1c level.

Already compromised kidney function due to renal calculi can be preserved with
1) Good control of blood sugar, blood pressure, cholesterol level.
2) Avoiding nephrotoxic drugs and food like pain killer medicines (NSAIDs - Non Steroidal Anti- Inflammatory Drugs).
3) Controlling metabolic parameters which are responsible for stone formation.

Pain due to renal calculi.

Detailed Answer:
Your pain is most likely due to bilateral renal calculi.

You can use pain killer tablet for pain relief.

But you should keep in mind that long term use of pain killer of group NSAIDs (Non Steroidal Anti- Inflammatory Drugs) like Diclofenac, ibuprofen deteriorate kidney function and raise serum creatinine which may be irreversible.

Use of pain killers like paracetamol, tramadol is safe for kidney.

It is important to go in to details of cause of renal calculi rather than just treating complications of renal calculi.

i am taking 5 leaves of neem tree, 10 glass of water, green tea one glass, 3 kms of walking, and avoid sweets, rice, oily food is enough for controlling blood sugar in fasting 132 or any other idea?

Whatever you are following is appreciable.

But single fasting blood sugar level of 132 do not always suggest good blood sugar control.
You should check postprandial blood sugar level and HbA1c level as well.
HbA1c level denote blood glucose control over a period of 3 to 4 months.

Depending on it appropriate anti-diabetic medicines can be advised.
