Question: Hello

I am 35 year Old Male, I weigh 107 kilos and 180 cm tall. Nearly 4 weeks back i had an RIRS + URS procedure on my left kidney to remove a stone. After 5 days of URS was performed on me doctor did another procedure to remove the stent, i will like to mention that I was on I.V. Antibiotics for whole five days . After Doctor removed stent i was put on oral antibiotics for 3 days. Both the procedures were done under general anesthesia and i was completely asleep during both the events After Stent removal i had no blood in urine or any difficulty in passing urine. But there was moderate pain for 4-5 days of stent removal in my left flank esp kidney area. I can clearly feel that something strong has been done with left kidney areas. After 5 days pain gradually went away, but i still have the sensation of endoscopic procedure in my left kidney area till date . There is also strong pain in my lower back (spine and hip joint ) till date

Now, After three days of oral antibiotics dosage completion ( after the stent removal) i developed fever of around 100. I had strong urge of urination as well. But there was no blood in urine or any nausea associated with fever . Doctor ordered a Ultrasound, a Blood CBC and a urinalysis test . All the three tests were negative . Doctor still put me on combination of I.V. and oral Antibiotics from the day fever emerged on a 5 day course . My fever went away on 4th day of the course. I completed my course and then doctor stopped every kind of medication and asked me to return to normal.

I will like to mention that since the onset of first procedure i developed mild Vertigo, Balancing Problems which increased significantly towards 3rd week to an extent that my urologist refered a neurologist who conducted a Head CT and some blood tests.

My head CT was negative. But i was adjudged with Dyslipidimea with high cholesterol and deficiency in Vitamin B 12 and D3.

So doctor provided be a blood thinner and certain supplements which i am taking for a week now. He also suggested that i do some physical exercise and i started cycling

I cycled for like 3 days (daily 20-25 mins). On 3rd after i started cycling (week after completion of my 5 day Antibiotic Course ). I again developed pain in left flank . I straight away went to my doctor .He said cycling has noting to do with it. and ordered a urine routine and culture. I got urine routine done (report attached).

Reports says i had 2-4 Pus Cells and 1-2 Epithelial cells, But doctor said it is normal. and he did not have me any medication. It has been 24 hours now since i developed this pain which is pretty significant and continuous now with same sensation that my left kidney has been "touched". Pain is at level where i can feel the discomfort . Please note doctor gave me same painkiller for lower back pain which was pretty consistent till i had the procedures . These painkillers have no effect on my Left flank pain .

I can feel around whole of my left flank? Is it ok ?
Are my tests normal ? If Yes then why i am having such a significant pain ?
What should i observe in my current condition before i rush to ER?

is it normal to have such kind of conditions?

I am also hypertensive and taking B.P. medications for 2 years

Thanks XXXXXXX

Brief Answer:
your symptoms are quite common after RIRS. and URS.

Detailed Answer:
Hello XXXXXXX welcome to HCM. your symptoms are common after a RIRS and URS.
there's no need to worry and your reports are normal. It's good that your vertigo has cleared and you're cycling regularly. As you know, your stone has been cleared with the surgery and DJStent was placed,to help reduce the inflammation and urinary infection, caused by the stone in kidney.now the left flank pain will gradually reduce in severity over the next 2-3 weeks, which is normal after endoscopic treatment of kidney stone.some anti-spasmodics may be prescribed by your urologist,if it's severe. wish you a speedy recovery.

Hello Dr. Mangat

I will like to clarify that my Vertigo has not cleared Yet.

Sorry, but if i understand you correctly you are saying that my current pain is spasmic in nature and it is normal that these spasms take 4-6 weeks to subside totally.?

And that my current report indicates that i have no infection ? And i can cycle without hesitation.

Your words are indeed comforting. I have one follow up question as well.

My Job requires me to travel internationally frequently. Do you think i should wait for while to be totally fit and then seek travel ?

Thanks XXXXXXX

Brief Answer:
wait for vertigo to clear and free of flank pain.

Detailed Answer:
Hello XXXXXXX You should wait till vertigo clears before travelling. The flank pain will settle down gradually. Drink 2-3 litres of liquids per day. See your Urologist, if flank pain is severe only.
Wish you a speedy recovery.
