Acute onset testicular swelling (commonly associated with pain) can be due to infection (Epididymoorchitis) or testicular torsion.

Both of these conditions require medical attention as soon as possible. If pain, redness is present along with swelling follow up with urgent care.

Slow onset testicular swelling unaccompanied by pain or redness is likely hydrocoele, hernia or testicular tumors.

If that is the case, it is not an emergency and he can follow up with the primary provider at the earliest.
