Question: Gallbladder is distended and shows few calculi mesasures 5.6mm, gallbladder wall is echogenic and thickened upto 6mm.
Impression
Acute on chronic calculus cholecystitis. FAtty liver grade 1.

Detailed Answer:
Hi
In cases of Acute on chronic cholecystitis , I prefer medical management initially. I Start with Antibiotics like Cefuroxime 500mg twice daily, Anti inflammatory tablet twice daily for 7 to 10 days.If there is severe pain, I give Intravenous Antibiotic and pain killer injections.
Once the Acute attack subsides,I will do Surgery and remove Gallbladder (Cholecystectomy).It can be Laparoscopically.
Regarding fatty liver, you should avoid oily food.Liv52 tablet twice daily or Liv52 syrup will be helpful.
Please let me know the values of Liver function tests.

Liver function tests are not very abnormal.

Hi
Except for mild increase in SGPT,other values are normal.
I still prefer Medical management initially.
Once the Acute inflammatory episode subsides,Gall bladder removal surgery can be planned.
