Brief Answer:
You need to start on Medicines for Diabetes

Detailed Answer:
Hi Mr. XXXXXXX

After going through your test results my interpretation is-
1) You need to start treatment for Diabetes.
There is family history of diabetes in your mom.
HbA1c level are increasing.
You need to start on Metformin in maximal tolerable dose which is first line drug for Type 2 diabetes.
2) Lipid profile is not acceptable for diabetic patient.
Recommended target of lipids in your case
For LDL cholesterol: below 100.
For Triglycerides level: below 150.
For HDL cholesterol: more than 40 (for male).
You need to restrict saturated fat content in diet, being diabetic you need to take cholesterol lowering medicines (statins).
3) Vitamin D level is in Insufficient range so it is advisable to continue Vit D supplement.
4) Vitamin B12 is in lower normal range you need to continue Vit B12 supplement.
5) HIGH SENSITIVE C - REACTIVE PROTEIN (HSCRP) is in High risk group (above 3.0 mg/L). High levels of hs-CRP predictive of an increased risk of a future heart attack, stroke, sudden cardiac death, and/or peripheral arterial disease.
6) HOMOCYSTEINE is also in high risk group (more than 15) It indicates possibility of blood clot formation and increases the risk of heart attack, stroke, Deep vein thrombosis. Taking Vitamin B12 and folic acid supplement will help.
7) Apo B/ Apo A ratio, Lipoprotein A have same significance as that of lipid profile, HSCRP.


Other tests are in acceptable range.
