What Do My Hormone Levels Indicate?

Question: Hi doctor.. I am a 30 year old woman of XXXXXXX origin.. Trying to conceive since 6 months.. I have regular cycles(28-32) days and my vaginal scans show I ovulate regularly.. But my hormone levels are a little out of place. FSH 8.04, AMH 1.3, LH 3.65 and prolactin 25.35 on day 2 of cycle.. what does this indicate? Will it be difficult to conceive naturally?

Hello mam, thanks for trusting health care magic.
Yes your hormones need active treatment and assistance for getting pregnancy as early as possible.
Your prolactin levels are high and AMH is low so best approach for you is to get ovulation induction and then in utero insemination (putting cleaned semen of your husband inside your uterus) after a proper follicular study.
But in case you are reluctant in approach towards any such treatment then you can try following measures to enhance the chances of conception -
1) try to keep a pillow beneath your buttocks after sex so that gravitational force keeps the semen inside vagina for a longer time.
2) you should not get up for at least 20-25 minutes after sex
3) try conventional position of sex female lying down on bed
4) maximum chances of conception are in day 10-18 of the cycle as you have regular cycles.
I hope these measures will help you in conception in near future.

Thank you Dr XXXXXXX I do follow all these steps already and will continue to do so. I wouldn't mind ttrying an ovulation induction, but I would prefer to try naturally for 2-3 more months before I consider an ininsemination. Why do you think an insemination may be necessary when I'm ovulating regularly?

I have recommended insemination because it increases the chances of conception manifold over ovulation induction.
Most important reason for advising it is your low AMH levels which will decline further with age.
So visit a good infertility specialist and try actively as soon as possible
But still you can try for 2-3 months and stick to the measures told to you

Thank You Dr. XXXXXXX . I visited a specialist yesterday and she put me on clomiphene tablets for 5 days this cycle with natural efforts.. She said we would try this approach to see how I would respond, and if not possible, do clomiphene injections with IUI next cycle based on our consent.. Coupled with your advice this seems like a good plan for us

Yes dear, you are safe hands as she is giving you a simple trial cycle of clomiphene which is an oral drug and will quantify the response to the drug and definitely there are chances that you conceive with that itself.
In next cycle clomiphene and FSH injections according to the response in this cycle along with insemination will give very good and predictable response and thus high chances of pregnancy.
