Question: Iam a female aged 24 of XXXXXXX origin based in California. In last 1 year have had cough (mild to severe) on several occasions. About 1 ½ month back I developed severe cough & cold symptoms probably due to exposure to cold. The doctor first gave only ceterizene saying it appeared as viral only but asked to contact again if not cured . This developed into more severe cough , fever , nasal drip & congestion, In next round with other physician, he informed that antibiotics will be needed . & also ordered a Chest Xray which was was Ok. Took cefixime 200mg+ Clavulanic acid for about a week & cough syrup . Fever went off in 3-4 days. Congestion improved but cough did not subside fully. About a week later also notices some rashes on different parts of my body which would appear, were itch & if nothing was done disappeared. Taking ceterizene reduced them. But general allergy to rubbing of clothes and rashes after hot water bath persisted for some time but have now reduced.
Have been XXXXXXX for last 2 weeks . had blood tests on advice of physician. Main points
WBC 13.42 (10^3 /uL)- Neut 71.2 , LYMP 22.9, MONO 3, EO-2.8, BASO 0.1
RBC 5.05 (10^6 /uL)- HGB-12.9(g/dL), HCT 40.4%, MCV 80 fL, MCHC 31.9 g/dL, RDW-SD 40.9 fL, RDW-CV 14.4 %
TSH (Throid) test results wer in range, Pulmonary function tests indicated spirometery within normal limits
Was prescribed azithromycon 500 mg for 5 days. & ebast-M (Ebastine+ montelukast) along with a cough syrup(Codeine Phosphate+Chlorpheniramine Maleate)
Also got a throat wab culture test before starting antibiotic on advices of a friend medical intern though did not wait for the results before starting azitrhromycin & above mentioned ebast-M tablet.
Throat swab culture indicated presence of Gram +ve cocci(staphylococci) .
Sensitive to cefroperazone/sulbactum, Piperacillin/Tazobactam, Meropenem, Moxifloxacin, Gemifloxacin, Levofloxacin, Ceftizoxime, Tobramycin, Sparfloxacin, Imipenem/Cllartatin.
Resistant to Ticarcillin/Clavulanic acid, Cefpirome, Cefpodoxime, Ceprozil.
However after taking this dose I have not been cured though I should admit I was exposed to cold also. Iam again faced with a coated tongue & increase in cough frequency with runny nose & some mucus.
Please advice possible treatment options & medicines & any other test needed. I want to be ok before I leave for USA from XXXXXXX in next 7-8 days time.
Regards, XXXXXXX

Brief Answer:
Antibiotic therapy need to be appropriate.

Detailed Answer:
Hello, XXXXXXX

I can certainly understand your concerns.

I apologize for the delayed response owing to my work schedule.

You probably seem to have inappropriately treated upper respiratory tract infection.

Since, it did not respond to Azithromycin and which showed sensitivity to Moxifloxacin on throat culture, it is important to start Moxifloxacin 400 mg per day single dose for 5 days.

You can continue with the remaining(latest prescription except Azithromycin).

This should offer you good response.

Please check with your physician if he shares my view and if can prescribe the same to you.

Thank you.
