What Do Dry, Chapped Lips With Acne Under The Lips Indicate?

Question: Hello doctor,

this is nxxxxxxxa , i have been battling with severe dry peeling lips for more than 2 years and after consulting doctors in this podium i have started using sebamed lip balm ....for the past one week i am on a business your to dubai ( i live in XXXXXXX UK) ... i do not know whether its because of the climate or not ... there is severe dryness in the lips and whenever i wet my lips with water or any food immediately a thin white layer forms which can be peeled .... it looks very ugly and it catches everybody's attention .... i am not able to bite it off also and until i use a cotton bud to wipe off the white layer it is there .... if i rub off the white layer the underneath skin remains red ( not that red but still yes it is ). i am applying sebamed lip balm every often but its seems to be not enough or whether i am having any other problem i do not know . also there are acnes coming in the chin under the lower lip . it hurts sometimes other times it itches .... its like one comes and almost wears off the other one comes close to it .... i do not touch it unless i am washing my face or moisturising it .... its been a week since i started using neutrogene deep clean face wash and followed by boutique anti acne water based moisturiser.... even while going out i apply cetaphil oil free moisturiser. i do not know why is it because of .... its embarrassing and painful ... i am sharing pics please help i am scared.

I am dr. kakkar (dermatologist). I have gine through your query and I have viewde the Images.

Perhaps you have a condition known as, "Cheilitis Glandularis" in which there is swelling and inflammation of minor salivary glands of lips and eversion of lower lip mucosa that results in obliteration of mucosal vermilion interface. With externalization and chronic exposure the delicate lower lip mucosa is secondarily affected by environment and sun exposure leading to dryness, erosion and crusting. There is no definite treatment for cheilitis glandularis except that you may continue with the sebamed lip balm and protect it from sun (preferably use a lip balm with SPF).
For recurrent acne on chin I suggest you to take a capsule of doxycycline 100mg, twice daily for 2-4 weeks and topically you may use clindamycin 1% gel, twice daily. These are prescription medications.

Hello doctor,
Today evening I happen to slip in a stream of running water in a park . The water level was not that much to cover even full off my foot but the floor of the running water was very slippery and my leg slipped and I fell down ... my main worry is I think some water splashed on my face including eyes and lips ... the water though it was running water not still water it was dirty with people putting their legs immersed in it and throwing garbage ... will I get herpes or any other deasies from it... I am soooo much so scared... it was just until few days ago that after consulting with you I consoled my mind not to get panicked and worried about herpes or any other diesease but now after the fall my fear is heightened ... also there are so many pimples that are appearing in the same area in the lower right side of the chin please suggest what should I do .... also please let me know if there is any abnormalities in my lips

No, there is really no an risk of acquiring herpes from this incident. As I already told you that herpes is only acquired through intimate contact like kissing and oral sex, not through a stream of water in a park or through other indirect means/ fomites.
For acne I suggest you to use either topical clindamycin 2% or benzoyl peroxide 2.5% gel, twice daily. In addition i also suggest Oral antiacne antibiotic like either doxycycline or minocycline. These are prescription medication but I don't think you will face any problem getting those in UAE as there lots of pvt hospitals with no waiting time and lots of XXXXXXX doctors there.
For lips I suggest you to just continue a SOF containing lip balm. Cheilitis glandularis is a chronic condition and treatment is only symptomatic.

Hello doctor,
U said about chellitis glandularis can you please tell me is it a disease or some skin issue or what exactly it is and how to cure it ... also will removing upper and lower lip hairs bring down the oilyness from that area thereby reducing the acne appearances ... I also have lot gums problems like periodontal and bleeding gums can this sometimes cause this slight swelling in the lips . I have attached few pics now can you please tell me if there is still any swelling or redness as such ...

Regarding your concern related to lip and oily skin

Detailed Answer:
Hi.

In cheilitis glandularis there is swelling and inflammation of lower lip leading to eversion of lower lip mucosa, in turn resulting in exposure of delicate lower lip mucosa to the environment. Symptoms include dryness, peeling, erosions and crusting. There is no definite treatment for cheilitis glandularis except keeping the lips hydrated and protection from sun with a lip balm containing SPF. Topical moderately potent steroids like mometasone furoate 0.1% or betamethasone valerate 0.1% may be tried and may help to reduce swelling and inflammation and associated symptoms.
Oiliness of skin can be better addressed by either using an astringent/ toner (witch XXXXXXX based) after cleansing. Use a gentle cleanser for face wash e.g cetaphil cleansing lotion, twice daily. Another option to address oily skin is Oral isotretinoin 5mg capsule, once a day. Hair removal is not going to reduce facial oil.
Gum problem is entirely a different issue altogether.

Hello doctor,
I have attached few latest pics could you please let me know if there is any sweeping or inflammation or nah redness ... I am washing my face twice or thrice daily and apply moisturiser and even apply lip balm two to three times a day even if I don't go out. With this chellitis problem that you mentioned can you please tell me any mouthwash and paste that I can use and still have no issues.

In few of the Images I can see some redness on the lower lip vermilion. I suggest you to use a lip balm which has an SPF. There is no recommendation regarding any particular mouthwash or paste. Just protect your lips from getting dry (with a lip balm ) and from sun exposure (with a lip balm containing SPF).
