Question: black red hard crusty blood discharge right nostril for months. nasal snot is clear has been going on for 6 Plus months had strep back in October had bronchitis in December 30th 2016 on Keflex on the 11th of January had sinus ear infection diagnosed buy a physician's assistant or nurse practitioner on Augmentin for 10 days. continuing to have great pain in face particularly right side behind eyes. watery right eye. Have seen dentist have seen endocrinologist have seen Primary Care. White hard discharge from right nostril once in awhile when sneezing. not having allergy symptoms. have passed prescriptions for prednisone and asthma medicines but no current asthma problems face extremely painful on the right side additional info is that I have 9 walnut-sized nodules on my thyroid tested each year via ultrasound but nine but a new one every year. recent blood tests are all normal although a little low in vitamin D. have seen asthma specialist 20 years ago but nothing current as Allergies have been under control. Allergic to floxin sulfa drugs aragosta and erythromycin. had Stevens-Johnson syndrome from floxin back in the nineties. back drainage excessive also trouble swallowing have to sleep in a sitting up position face really hurts on the right side especially behind the eyes have seen a doctor eyes are okay teeth okay

First of all, it is sad to know what you had been experiencing lately. Based on the history you have shared, it seems as if you have already been through alot. At this moment most important thing which needs to be assessed as soon as possible is the suspected infection of Paranasal sinuses and it's extensions in the oral or sorrounding structures in head and neck.

You need to have a detailed ENT examination or otolaryngologist review alongwith radiological investigations like CT or MRI of Paranasal sinuses and head and neck to look for extension of infection.

Also it may need to be evaluated in cultures and sensitivity of swabs taken from the nasal discharge and basic blood work up including blood counts and ESR and CRP.

Based on the history of steroids, immunocompromised state followed by that may lead to increased susceptibility to acute viral or fungal infections as well. And due to history of severe allergic reactions to antibiotics in past, options are very limmited and may need penicillin antibiotics like augmentin or cephalosporin antibiotics like cefixime or cephradine orally or injectable options like ceftriaxone or tetracycline but that should all be preferably chosen on the basis of cultures and sensitivity reports.

Meanwhile analgesics like acetaminophen and anti histamines like cetrizine or loratidine can be used for symptomatic relief. Healthy lifestyle like regular exercise and hygienic food and cleaner drinking water will help.

Thank you. I saw doctor as recommended. She ordered a CT scan and is giving me a referral to ENT specialist. She also recommend a second opinion from an endocrinologist as each year for 4.5 years I get more thyroid nodules have 9 walnut or golf ball size nodules that the yearly ultrasound shows. Gained 25 pounds since Oct when sinus pain on right side started to be so very bad and deep blood discharge. Working full time and have to wait until off work each day to take hydrocodone 5/325 which gives some relief. Right eye waters a lot. Got blood labs and low on vitamin D and loss of some bone density they told me. I am unsure if all the T tests were done. Would warm packs on my face help or hurt? Face right sinus areas hurt and behind eye. No illegal drugs no alcohol no smoking. Doctor said right ear has wax not the left. Back drainage.Sincerely

I would suggest to proceed with the CT and ENT review as soon as possible. Meanwhile analgesics which you are using can be continued. But decision of antibiotics should be made after the CT report. The endocrinologist review and fresh Thyroid levels and decision of biopsy of the nodules should be made as soon as possible. The vitamin D replacement and calcium supplements should be continued too. Warm compresses may help temporarily but long term solutions need to be sorted out and sooner the better. Regards.
