What Could Cause Low Grade Fever For Over 10 Days Despite Taking Antibiotics?
Question: I am having low grade fever , range from 99.0 F to 100 F ,oral, as against the usual limit of 98.6, on healthy days ,for me it was around 97.8 to 98.1.It means I have low grade fever for last 10 days. There are absolutely no other symptoms of bacterial if viral infection ,no sore throat etc... With this small fever ,I feel too tired.all my blood test incl CRP etc is absolutely normal.recent chest x Ray and whole abdomen ultrasound is normal, except that chest x Ray says, both lungs fields shows broncho vascular markings ...earlier my blood test report has low TLC of 3700 but blood test now says it is 4200.all other typhi test, malaria, dengu etc is normal.i took antibiotic cefaxime 400 mg twice for 4 days and Azee 500 Mg once daily for 5 days..
I am worried on my x Ray report as above
..also the x Ray of paranasal sinus says ,thickning of inferior turbinates is seen, impression Rhinitis.i don't have any lungs congestion or pain...also I don't have any nasal blockage or mucuous.

I would like to comment the following:
1. You seem to be having fever since 10 days with no other apparent symptom.
2. Your Blood counts seem to be normal. You do not have any current chest symptoms.
3. The chest x-ray finding of prominent bronchovascular marking has relevance only with chest symptoms like cough if present and in such a scenario may represent bronchitis.
4. A para nasal sinuses was done possibly for you having reported symptom like cold or nasal blocking.
5. Persistent fever despite short antibiotic course requires further evaluation to rule out systemic / infectious diseases.
6. Also a period of rest for 1 week may help you.
7. Please get yourself clinically evaluated by a physician.

I request to kindly check all reports and prescription as I uploaded...just now...May I request you to kindly see reports and PL advise...
No nasal congestion is felt , no mucuous, no cough and other symptom.During day with some household work, temp reaches to 99.5 XXXXXXX oral...again in the evening , morning etc temp remains below 98.6...
My question is ,after seeing all reports normal , except that case 19.9 is marginally high ,as per report, is there any cause of worry or still you think that CT scan would be required, which we are trying to avoid , if there is no need for it...I feel, even if I go back to a doctor , he will see these reports only and therefore I feel ,your advise will be same as going to a doctor...

May I add , is there any risk or further evaluation required in view of markings as shown in x Ray report...

2. The reports are suggestive of URTI or upper respiratory tract infection.
3. Since you do not have any chest symptoms at present, other causes of fever need to be investigated further by means of thorough clinical examination like detection of any enlarged lymph nodes and not necessarily investigations. Please be informed that medical science is not an algorithm based on investigations. Even though the CA 19.9 was reported marginally high, it needs to be clinically co-related. Detailed history and clinical examination are prerequisites to the interpretation of investigations and no investigation should be interpreted in isolation. Please do not miss your appointed follow up with your doctor.
4. Your ESR is raised which may be due to an infection in the body. However be aware that ESR is very non-specific. As advised the course of antibiotics may be continued for some more time by your physician if fever persists.
5. You need to avoid outdoors also as sometimes increased exposure to a hot environment outside could lead to elevated body temperature.
6. At this point, based on the data provided, CT scan does not seem to be the first investigation of choice in my opinion.
