Question: Sir, I have an infection at front part of my Penis. Small WOunds are on it. I have gone through medical Treatment for fungal infection also. But after some times problem again started. I am always using COmndom while sex. Herpes is also negative. Now currentyl ttreatment goes on is
Microdox LBX once a day
Leveocertrizine 5 mg.
T bact ointment,
When i have an sex the problem get worse.

I have gone through your query and I have understood it.

I will keep a possibility of candidal balanoposthitis. From the query it is clear that antifungal treatment has been effective in the past but the condition recurs again, specially after sexual contact.
Candidal involvement of glans and prepuce can be recurrent in certain common situations like: underlying diabetes, repeated infections from sexual partner.

I would like to know more from you regarding the query.

-Does the condition involve prepuce predominantly or both glans as well as prepuce?
-Have you ever tested for blood sugar?
-Do you always use a condom?Or is it sometimes unprotected as well?

Sir I always use condom, never without condom any time from last 8 years. I think this may be due to friction. When some time it erects in trousers, It get frictioned and probelm increases. My Blood sugar level is normal. I am usinf vaseline as a lubricant on my front shaft of penis.,. WIth this some relief otherwise problem increases, Now from last 20 days i am continue using Microdox LBX & Cetrizein. But problem still stands. What is permanenet solution for this. Is there need or blood culture or any other test. HIV, Herpes is normal.

-This problem can also arise in those who have a tight foreskin. Are you easily able to retract foreskin?
-How do you clean that area? Do you use soaps?antiseptic washes etc?

My fore skin is normal i can easily move it on head. I am not using any soap or antiseptic for washing penis area. SImply clean with fresh water. SIr, Onething more i have also problem in backside 2 years back. Crack in skin at bottm heaf of spinal bones. 2-3 month back whitish material also comes. Photo has been uploaded. Also mailed to your given mail iD. I already opladed photo in previous follow up also. 2 photos has been uploaded in report section.

Possibility of Erosive Lichen Planus

Hi.

Yes, I have reviewed the photographs.
I can see a few erosive lesions/raw areas on the left side of glans penis.
After seeing the photographs and carefully going through your history and ruling out possibilities like candidal infection and herpes, I would like to keep a possibility of "Erosive Lichen planus".
Topical steroids work for Lichen planus e.g mometasone furoate 0.1% cream, can be used twice daily for 2 weeks.
It can present with oral mucosal erosions as well along with genital erosions but may occur in isolation.
This is a prescription product and I would suggest that you revisit a dermatologist in your region for a confirmatory diagnosis as well as the needful.
The cream that you have been using is a topical antibacterial and won't help much with this condition.

Ok I will go through the cream that you have suggested is any need of tablets to be take what to take for candida infection . microdox lbx should be continue or stop
near by demartologits suggesting same treatment from lat 1 month that has been mentioned

Hi.

I would suggest only this cream, twice daily.
No tablets except that you may repeat a "Single dose" of an oral antifungal i.e fluconazole as before.
You may also stop the tablets that you are already taking.
