I can see erythematous/ red patches of thick skin on scrotum where it is attached at the base of penis. This is scrotal Lichen simplex chronicus (LSC). This is not a fungal infection.

LSC is an itchy skin condition wherein the affected skin turns thick with prominent skin markings (manifested as increased rugosity of scrotum), is dry, scaly and red. In later stages the affected skin appears darker/ hyperpigmented as compared to surrounding XXXXXXX skin.
LSC is a response to chronic itching and scratching of skin and this thickened skin stands out distinctly from surrounding normal skin.

LSC is more common in those who are atopics i.e have a dry skin.

Treatment of LSC is with Oral antihistamines e.g once or twice daily cetrizine tablet Or levocetrizine tablet, to abolish the itch stimulus.
Topical moderately potent steroids e.g mometasone furoate 0.1% cream Or fluticasone propionate 0.05% cream, twice daily for 2-4 weeks is also helpful in this itchy skin condition.

A moderately potent topical steroid

Hi.

You may use one of a moderately potent topical steroid cream e.g fluticasone propionate 0.05% Or Triamcinolone acetonide 0.1% Or Mometasone furoate 0.1% Or Betamethasone valerate 0.1%.

I am not sure whether you would get any one of these otc in your country. You can try to find one of these from a drug store near you Or else you may visit a doctor in your vicinity and he/she may prescribe one of these.

In addition I would also suggest an otc oral antihistamine e.g cetrizine tablet for symptomatic relief from itching.

Can LSC be cured? Can i take like terrasil stop itching cream without those steroid cream? Is this a serious skin desease?

Hi.

Yes, LSC can be cured.
As I mentioned earlier, LSC develops in response to chronic scratching and rubbing of skin. Therefore the most important part in treatment of LSC is to control itching and the desire to scratch.
This is usually achieved with daily oral antihistamines e.g cetrizine tablet Or levocetrizine tablet. Cetrizine is available otc.
You must take a tablet of an oral antihistamine i.e either cetrizine Or levocetrizine once daily for 2-4 weeks. Not scrtaching that area would itself help reverse skin changes over a period of time.
Regarding topical creams, only steroids based creams are effective in LSC.
