What Causes Vaginal Itching?

Question: This is concerning a friend of my age. She constantly gets an itchy feeling near her vagina (externally just in the pubic region, in the area that remains moist). Sometimes she suspects it is extra white discharge sometimes nothing. She also maintains a decent level of hygiene and cleans herself after voiding urine etc. The problem has been persistent for a very long time and she has found no reasons. It gets rather embarassing at times. Kindly advise. Also suggest if there are any specific hygiene measures to be taken.

Hello,
Thanks for contacting health care magic.

Well going by your description of the nature of vaginal discharge which is thick and white and she has itching, it indicates vaginal candidiasis.

She must consult a gynaecologist for a per speculum examination which would allow for confirmation of diagnosis by a swab culture.

If the culture grows fungal hyphae then the diagnosis is confirmed. Otherwise clinically it appears to be Candidiasis , ie, fungal infection.

If you would have been my patient at the clinic I would have advised you to take TAB FLUCONAZOLE 300mg STAT. This drug is not available over the counter, and hence you would have to seek a prescription from your gyanecologist for the same. Simultaneously your partner would also have to be treated with the same dosage and the drug to avoid inter-infection.

Also take CLOTRIMAZOLE VAGINAL PESSARY to be placed intravaginally every night before sleeping for 6 days. Also to avoid intercourse during the treatment.

Hello.

Thanks for the follow up.

Well, the symptom of itchiness itself points towards fungal etiology and you said it was white.

There are other types of discharges caused by different organisms like chlamydia or trichomoniasis or the most common bacterial vaginosis. In all of them itching is present in trichomoniasis only but it has a greenish discharge and foul smell.

If the discharge is not prominent and the itch is over the vulval area and not intravaginal then you may use ointment clotrimazole for local application thrice a day instead of pessaries.

The itching is only external in the region around the vagina & not intra-vaginal. So should I go ahead with the ointment?

Tab fluconazole can also be helpful but has some side effects. You may get it prescribed from your gynaecologist.
