Question: i have continous urge to urinate low fever and general malise. i am male 50 yrs. just returned from a 10 day trip outside my city

I have read through your query in detail.
Please find my observations below.

1. It is possible that you have acquired a bacterial urinary tract infection during your trip out of the city which was for 10 days.

2. During your trip, you might have used a public toilet which was unclean due to shortage of water and unhygienic conditions, you might have got an infection.

3. Please do the following for your suspected urinary infection.
(i) For fever you might have to take paracetamol 500 mg tablet 6 hourly as required.

(ii) For any burning or pain during urination please purchase an anti spasmodic medicine like HYOSPAN or HYOCIMAX tablets 10 mg which is an over the counter medicine from a pharmacy or medical store. Please check expiry date when buying and take in recommended dose of 20 mg 2 to 4 times a day only if there is spasmodic pain.

(iii) It is recommended to give urine sample for culture and sensitivity test (results will take 2 to 3 days)

(iv) Drink lots of water and fresh juices to flush your system.

(v) Consult your doctor with urine culture and sensitivity report and your doctor will tell you if you have any bacterial urinary infection and decide which antibiotic is best for you. Please take complete course of antibiotics.

(vi) Your doctor might also tell you to get an ultrasound scan to know if there is any problem with kidneys or urinary bladder.

(vii) Please avoid touching dirty surfaces when using public toilets.

1. Antibiotics are powerful medicines and to be given with prescription only after consulting a doctor. We do not give antibiotics prescriptions online as per ehealth policy guidelines.

2. Taking antibiotics before getting urine culture and sensitivity is a wrong concept as it might not treat the infection completely and this can be a problem later on which can become dangerous to your health.

3. In the best interest of your health, it is suggested to first give urine sample for culture and sensitivity test and then take antibiotics after knowing which antibiotic will work for your infection.

1. Your case is under stood and if you were my patient and approached me at the hospital with your problems then I would have suggested the following as an emergency treatment.
(i) Take a sample of urine for culture and sensitivity test.

(ii) Suggest drinking lots of water and syrup ALKASOL (disodium hydrogen citrate) to be taken 1 to 2 teaspoon full 2 to 4 times daily diluted in a glass of water. This will normalize the urinary tract condition and make it unfavorable for bacteria.

(iii) Till the time the culture results come I would have put you on antibiotic URIFAST or URIBID (nitrofurantoin) 100 mg 4 times a day for 7 days. Please note that this is strictly a prescription medicine and should be taken only after consulting a doctor and for confirmed urinary tract infection. This is to be taken only if kidney functions are normal. Culture reports however are required to verify if this antibiotic will work or not.
