What Causes Upper And Lower Abdominal Pain?

Question: My husband has been having upper and lower abdominal pain for the past 4 months. He is 45, 5'10 and 260lbs, never used drugs, alcohol or smoked. We went to ER for abdominal pain and chest pain in Aug, but they only focused on chest pain. He had an episode of a-fib while we were there (he has whitecoat syndrome), they kept him for 3 days, medicated and gave 2 shots of blood thinners in his lower abdomen. We followed up with cardiologist - she said everything is fine and that he will never have another a-fib episode again, he's on blood pressure meds, had stress test, several echo's and ekgs - all fine. Then we saw gastroenterologist - had abdominal sonogram, ct with contrast, hida scan, upper gi, and colonoscopy -all fine. Still has abdominal pain in gall bladder area, in appendix area, in his belt area and in spleen area. Even went to a new PCP but he doesn't seemed concerned at all, just did non-fasting bloodwork - came back fine. Pain is constant, ranging from mild to severe at times. He also takes bentyl for spasms, but they do not work. He was completely healthy and active until August when this all started. I am at my wits end because no doctor can tell me what is causing his pain, and he has been suffering for FOUR months.

It is a pain in the abdomen with unidentified cause though all the investigations are OK. You need to describe the character of the pain, whether it is dull or acute , constant or spasmodic. Does this pain have any association, I mean what are the aggravating or relieving factors. They may give some clue about the nature of pain.
At this stage your husband needs reassurance, a fibrous non constipating food, and good hydration so that he has free flow of urine. I wonder if he has a small stone in the urinary tract not really picked up on USG which is slowly migrating and giving rise to spasmodic colicky pain. Any way with adequate hydration with alkaline supplements like dihydrogen sodium citrate in it and antispasmodic drug when needed should in a short time relieve him of this chronic abdominal pain. Your personal physician will advise you on this.
