Posted on Sat, 19 Apr 2014
Question: I have pain in my upper abdomen and lower chest. It is not constant but when it comes on it is very painful and lasts from 1/2 hour to 1 1/2 hours. It happens several times a day and lasts longer in the evening before I go to bed. It doesn't seem to be related to anything I eat. It has been going on for 3 months now and gets worse by the day. My doctor has scheduled an Endoscopy....What might he be looking for?

Brief Answer: To rule out acid peptic disease Detailed Answer: Hi Welcome to Healthcare-Magic Greetings of the day Dear Ms Pain in upper abdomen and lower chest which occurs several times a day, especially with no relation with food I would consider the following possibility 1. Gastro-oesophageal reflux (GERD)with or without associated Hiatus Hernia: Presence of GERD could present with abdomen pain, especially in the epigastric region and retrosternal burning pain. If there is assocaited Hiatus hernia, it could present with pain in abdomen and chest with no relationship with food intake. GERD is usually diagnosed by upper gastro-intestinal contrast study and 24 hours Ph study. Hiatus Hernia can be easily be picked up by Endoscopy 2. Acid Peptic disease: Acid peptic disease could also present with such symptoms , especially Gastritis and Gastric Ulcer 3. Oesophagitis: It also requires Endoscopy for diagnosis. 4. Narrow Gastro-oesophageal opening: Assessment of tightness of lap band will require Endoscopy Its a clinical dictum/recommendation that any individual who presents with recent onset of Upper gastro-intestinal complaints , its important to subject the individual to Endoscopy. I assume that the CAT scan done in your case , Gall bladder pathology as the cause for symptoms has been ruled out. I hope it helped. In case you need any further assistance, will be glad to assist you. Take Care Wishing you happy and healthy life. Best Regards Dr Deepak Kishore MBBS,MS,MCH Consultant Surgeon

Someone..not a doctor....told me it could be non-hodgkins lymphoma because of those symptoms and I feel bloated much of the time. Can I rule this out or any kind of stomach cancer?

Brief Answer: Unlikely to be Non Hodgkins Detailed Answer: Dear Ms If it was Non-Hodgkins Lymphoma which usually involves lymph nodes and abdominal organs it would definitely be picked up by CAT scan which you had a week back. Endoscopy is basically done to find out any pathology of Mucosal lining or lumen of Oesophagus, stomach and duodenum. Rest be assured, its highly unlikely to be Hodgkin's . CAT scan and a experienced Radiologist will never miss it. Wishing you a happy and healthy life ahead. Take Care Regards Deepak
