What Causes Upper Abdominal Pain And Bloating?

default
Posted on Mon, 2 Mar 2015
Question: I have been suffering from chronic stomach pain for the past 6 months after meals (home cooked or from outside, doesn't matter). the pain persists for a few hours and then subsides on its own. Occasionally I feel contractions in my stomach as well.
Passage of stool is regular and normal (once a day). Not much flatulence.
I have been to 2 gastroenterologists. One of them had conducted an endoscopy which is clear. the other had asked for a complete blood reports and stool micro test. The reports are again clear. Now the current specialist has asked me to undergo a colonoscopy on Thursday.
I am attaching the reports and prescriptions of both the doctors.

I would like to get a second opinion if I need a colonoscopy here and what might be probable ailment here.

Need more information

Detailed Answer:
Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

I would like to have some more information before I can give my opinion.

From the reports I can see you are having upper abdominal pain and bloating.
Your endoscopy shows antral gastritis and you are negative for helicobacter pylori infection.
Your complete blood count and stool tests does not show anything abnormal.
Your bowel movements are absolutely fine.

I would first treat your antral gastritis with 4-8 weeks of PPI with prokinetics like Iladac-L (taken once daily). You have taken similar medications for only 15 days.
Take the medication I have mentioned here for 4-8 weeks and see the response.

I will not advise colonoscopy now.

Regards,
Dr.R.K.

Thank you for your response Dr. RK.

I would also like to share with you that once I lie down on my back while it pains, it goes away within 10 minutes. Also, I occasionally feel some contractions in my stomach at different times. I also have irregular motions if I consume milk. There is more chance of developing stomach pain if the food is spicy.
Hope this may be diagnostically relevant for you....

Now, my second specialist here in XXXXXXX (Dr. Bhatt) has suggested that I may be suffering from Bowel Colic, while you and Dr. Lahoti (from XXXXXXX Hospitals, Delhi) have proposed it to be Antral Gastritis. I would like to understand reason behind the differing diagnosis and why you think Antral Gastritis is a better fit.

Also, if I go along with your recommendation, is it OK if I continue with the medications prescribed to me by Dr. Lahoti? Or do you propose any changes to it?

Many thanks for for your time and patience.

Best,
XXXXX XXXXXXX

Detailed Answer:
Hi,
I said antral gastritis because that is what the endoscopy shows.

Alright doctor. But do you see any reason in the reports or from description as to why Dr. Bhatt is suggesting it as Bowel Colic?

Also, do Iladac-L and Pantocid 40mg have similar effect and composition?
Can I continue with Pantocid for 4 weeks?

Brief Answer:
Take Iladac-L

Detailed Answer:
Hi,
I cannot comment on Dr.Bhat's opinion. You need to ask him only.

Iladac-L and Pantocid 40 are two different medications.
You can take Iladac-L for better control of symptoms.

Regards,
Dr.R.K.
