What Causes Unexplained Weight Loss Along With Swollen Lymph Nodes In Lower Back?

Question: i am a 49 yo female, past couple yr unexplained weight loss from 2011 winter thru spring 2012 I was 130 went down to 116. appetite eh, really none. ate and I felt too full fast. lower back pain seems like there are lymph nodes in my lower back my husband and I both feel them..i wake up at night itching all over for no reason. feet, legs, then sides of back, then arms hands..then it'll subside and not happen again for days. some nights wake up all sweated up.
how long before I should go see my pcp dr.? even my legs feel weak when I walk up our stairs to go upstairs to bedrooms, and I always shrug it off as ohh im getting older.. How long till you are able to give some sort of advice?

Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.

Based on your query, my opinion is as follows:

1. You should meet your primary care provider at the earliest.
2. Enlarged multiple lymph nodes is not good. Especially when associated with sweating in nights, and weight loss, lymphoma is strongly suspected. Fine needle aspiration cytology (FNAC) of the lymph nodes will be helpful in further evaluation.
3. Itching at night could be due to dry skin. Apply moisturizers and avoid hard soaps.
4. Also get a complete hemogram for evaluation of anemia due to weakness.
5. X-ray of back will be necessary for evaluation of back pain. Muscle strengthening with back exercises and lumbar support braces along with posture training will be helpful. Do take calcium supplements too.

If those are not lymph nodes, or no signs of lymphoma is seen, malnutrition leading on to anemia and perimenopausal symptoms causing night sweats and hot flashes will require evaluation. Hormone replacement therapy will be helpful in this scenario.

Do meet your PCP for lymph node evaluation and complete hemogram. Based on reports, further opinion possible.
