What Causes Tiredness, Severe Nausea, Sore Throat, Night Sweats And Loss Of Appetite?

Question: Hello, I'm concerned about my daughter. She's 19, and has been complaining of tiredness, she's had bouts of nausea, soreness in the throat area some night sweats and a slight loss of appetite. This all started about 5/6 months ago. They found nodules on her thyroid. One came back benign and one was inconclusive even with FNA. We initially thought that it was low thyroid level, so we started her on synthroid. It seemed to help her at first but now not so much. We are beginning to wonder what else could be wrong because the symptoms seem to be getting worse. We have been to an ENT twice about her throat soreness but he seems to keep writing it off as nothing. All of her blood test have came back normal, but we don't know if they are checking for the right things. We do know something is very wrong and we are very concerned. What other possible things could this be a sign of? Please help.

I can understand your concern regarding to your daughter's health.
Of course a detailed clinical examination and the results of the blood tests is important.
There are many blood tests to be done and I do not know what she has done until now.
As far as I can understand from your description, she has been taking levothyroxine, the dose should be monitored, night sweating may be due to an overdosage.
For the chronic tiredness and nausea I would suggest to exclude a low cortisol production by the adrenal gland (if it is not done).
Did she check the temperature? Has it ever been high?
Does she has weight loss? Is the menstrual cycle regular?
Any family predisposition for any pathology?
Please try to attach the exams she has done until now and try to provide some more info.

Thank you for your response. No weight loss but interestingly there has been a slight decrease in appetite. And today, she ran a low grade fever. Im not sure what you mean by predisposition for pathology. Can you explain more on that? I do not have any exams to attach as we did not get a copy of the lab reports. Menstrual cycle is regular. This is all just really concerning as we went online and looked up lymphoma, both hodgkins and non hodgkins, and she has most of the symptoms so as parents naturally we are freaking out. We will not be able to get back to see her doctor until next week.

It is not a typical presentation of lymphoma;

Welcome back,
Family predisposition for pathologies, means if the parents has any disease that may be inherited.
Lymphoma usually is manifested with fever, exessive sweating, enlarged lymphnodes in clinical or radiological examinations and in most of the cases alterations in the whole blood count .
So her physician has excluded it.
If the sore throat would be due to low immunity caused by lymphoma, the white cells in the whole blood count would be low.
In very rare cases lymphoma is not manifested with fever, enlarged lymphnodes or alterations in blood count and is diagnosed only with myelogram. It is not her case (the symptoms you described are not suggestive).
Regarding to other possible diagnosis, it is difficult to say without clinical examination and the blood tests she has done until now.

Thank you Doctor. All i know is that her symtoms seem to be getting worse. She was diagnosed with thyroid nodules about 6 months ago. The endo seems to think they are not cancerous. He also think she does not need to be on synthroid mainly due the lab results. At first it seemed to help but not so much now. She has went from .25 to .50 on dosage. Now its like its not even working. The soreness around the throat and neck area were diagnosed by her ENT as TMJ. He did feel her neck and throat and did not notice anything except the nodule. No swollen lymph nodes or anything. But, during all of this she was having bouts of nausea and some night sweats. This was something she had not told me. Now the fatigue seems to be getting worse. If her symtoms are not suggestive of lymphoma, do you think some of the symtoms could be caused by too much synthroid? And what about her neck and throat soreness? Im starting to think now since we have has two doctors tell us that she does not need to be on synthroid, mainly due to her labs, coupled with the fact that its no longer working, that the synthroid may be doing more harm than good. I just dont know what to do or think. I wish we could take her in today for labs. Im glad to know you think this is not suggestive of lymphoma, but, what else could it be?

Regarding levothyroxine, it is not suggested to be taken if the thyroid function is within the normal ranges.

But I do not think the symptoms you reported are due to levothyroxine.

It would be better if you please upload/attach a picture of the neck soreness.

However, rheumatologic pathologies (like lupus erythematosus) should probably be taken into consideration too.

Thanks. I do nont have any pic of her neck/throat. Theres really nothing visible. She just complains of soreness and tenderness. While at work today she ran a fever and srarted to sweat again. This is a place where they keep it 61 degrees. The bout did not last very long she said. I think she also mentioned a touch of nausea. Do you think we should go straight to the Endo or take her to her PCP first?

If there is nothing visible and is a matter of only 1 day nausea and fever, you should not get worried.
You may take her to PCP first, a proteine C reactive over the normal ranges would need further exams.

The bouts of nasusea and sweating have been happening for about a month or two. Not sure about the fever. These symtoms mostly started around the time she started taking the synthroid. The fatigue and throat/neck soreness were present before starting the synthroid. Since they have done CT scans and sonagrams of her throat and have not found anything but the nodules, it is possible that these nodules are causing her throat discomfor? Also, is it possible that her symptoms are just side effects of the synthroid?

Hi again,
Exessive sweating and weight loss mat be due to synthroid ( even though the dose she is taking is low) , but sore throat, nausea, fatigue can not be side effects of synthroid.
There is a viral infection of the thyroid gland (subacute thyroiditis) that causes pain in the neck, fatigue that last up to 6 months, but usually the eritrosediment is high so the blood tests wouldn' t be normal. If the thyroid nodules are not active ( causing hyperthyroidism) and malignancy is excluded by FNA, they do not cause such symptoms.
Citomegalovirus may be also a cause of sore throat and fatigue, also associated with elevated protein C reactive and ESR.
That is why is difficult to judge without knowing the results of the blood test.
I am sure there is nothing serious, probably an anxiety disorder but there are some pathologies (some of them mentioned above) that should be excluded before.
