Question: Yes, I have been having tingling in my hands and feet for a few weeks. More recently I have had diarrhea, a none steady urine stream with some burning, smelly urine, lack of blood flow to penis, I cannot get an erection, and abdominal pain. I have been very tired and my eyes feel heavy. Feels like my fingers and toes are going to sleep all day. Any advise would be great.

Brief Answer:
Serum vitamin B12 levels must be tested.

Detailed Answer:
Hello
Welcome to HCM.
I can understand your concerns.
I have few suggestions for you:
1. The tingling sensations and numbness in your limbs can be due to some medical reasons. Most common are diabetes and vitamin B12 deficiency. Other common reason is cervical spobdylosis or lumbar disc prolapse. Please get evaluated by a physician for these causes.
2. The changes in urine can be due to urinary tract infection. If the urine examination is inconsistent, you can go for ultrasound abdomen (as you are having pain in abdomen) and kidney function tests.
3. The blood flow to penis can be tested by penile doppler study.
4. You can take multivitamin supplements for tiredness.

Hope this helps you.
Any further queries are welcome.
Thanks.

Brief Answer:
There could be some medical disorders.

Detailed Answer:
Thanks for the follow up query.

I could think of few possibilities like
Diabetes,
Hypertension,
Vitamin B12 deficiency,
Cervical/lumbar spondylitis.
And
Renal infection or stone for urinary problem,
Some gastritis for abdominal pain.

The investigations which I have listed earlier can help you to rule out these diseases.
I would suggest you to visit your nearest physician for further evaluation and management.

Thanks.

I forgot to add that I have also stopped drinking alcohol significantly as of the last month. I have dropped over 10 lbs in 2 weeks. Is that alarming?

Answered by Dr. Shubham Mehta (10 minutes later)
Brief Answer:
Its not alarming.

Detailed Answer:
Keeping in mind the alcohol addiction, its really important that you should get your liver functions tests done alongwith ultrasound of abdomen.
However, i dont think this weight loss is alarming. You should be worried if the weight loss is continuing, the appetite is poor and the liver functions are deteriorated.

Hope this helps.
Thanks.

I apologize, I keep coming up with more issues I've been having. It hurts to walk now, I feel a pain in my scrotum. This morning my knees locked up while my legs felt numb. Never happened before. I have white blotches on my arms, perhaps just pigment loss. I am having a hard time passing stool now, straining with no bm. When I do pass stool it is brown/green and very stringy with mucus material after whipping. I lost 4 lbs in two days. I was 200 lbs just two days ago. Any more thoughts. You probably think I'm a crazy person.

I realize you are a psychiatrist. Do you know a lot about these issues?

Oh and I have been belching out of control all day. Can't stop, very strange and new.

It seems there is a lot of fluid buildup in my scrotum and both scrotum and penis are inflammed and aching.

Brief Answer:
Visit a general surgeon.

Detailed Answer:
You can ask your queries without hesitation. No need to apologize :)

I can understand you are having several health issues.
First and foremost advice now would be to visit a general surgeon. The inflammation and accumulation of fluid in penis and scrotum need urgent surgical intervention.

Your gastric symptoms like belching, constipation and change in colour and mucus in stool is most probably a liver or gall bladder pathology. This will also be handled by a general surgeon.

The locking up of knee may be due to meniscal tear. But you can take a tablet of combiflam to relieve pain and inflammation.
White patches on arms can dealt later by a dermatologist.

I insist you to visit your surgeon for penile and scrotal inflammation and gastric issues.

Hope this helps.
Thanks.
