Brief Answer:
Various causes of earache needs to be ruled out...

Detailed Answer:
Hi

Thanks for posting the query. I will try my best to help you with this query.

There are various causes for throbbing earache. To name a few:
1. Eustachian tube catarrh - Eustachian tube is a tube connecting nose and the ear. Any block noticed in the nasal end of the tube can increase pressure in the middle ear thus causing pain.
2. Otitis media - Middle ear infection secondary to the Eustachian tube catarrh can also cause earache.
3. Otitis externa - inflammation of the external ear.
4. TMJ dysfunction - Temporo-mandibular joint dysfunction of the affected side is one of the reason for throbbing earache.

Since you are already on 4th day of medicines (antibiotic perhaps), I will urge you to report back to the doctor to isolate the cause. It requires good clinical examination of the ear, nose and throat before diagnosing the actual cause, and the management can be planned accordingly.

Meanwhile, try taking OTC pain relievers such as ibuprofen. Consult your doctor at the earliest.
