What Causes Testicular Pain With Pain In Lower Abdomen After Urination?

Question: Hi

I have recently been getting pains in my right testicle. I have received a few kicks to the testicles a few years ago, but not a major injury.
I have noticed that my scrotum skin has become red and I get pain in my lower abdomen after urination.
I was born with problems with my kidneys and had an operation to correct bilateral reflux when I was 6 years old. I currently have right sided renal scarring.
Could this possible be linked and should I see my GP. What would he do to examine me and what kind of tests would I need?

I appreciate your concern

Pain in lower abdomen after urination and painful right testicle both indicate a genitourinary cause. In addition, history of surgical procedure for the reflux correction warrants that investigations be done to rule out any complications.

Based on your presenting complaint, your GP would first do a general physical examination looking for general signs of concern like anemia, jaundice, any edema, fever. Then he would specifically examine your abdomen and testicles after your permission.

Urinalysis, Urine culture sensitivity testing, ultrasound abdomen and pelvis and scrotal ultrasound should be advised. Blood counts, ESR , electrolytes and renal function tests should also be done as part of the workup.

1. It can be a urinary tract infection, epidydmitis, orchitis. A course of antibiotics and NSAIDs can be advised accordingly for treatment.

2. Both abdominal and scrotal ultrasound are extremely safe procedures. They are not invasive. The radiologist applies gel on the area to be examined and puts the ultrasonic probe on the area to look for any abnormality. You will feel no pain during this investigation.

Hello

I visited my doctor today regarding my problem and he examined it and found a cyst in my epididymus. He said they are quite common.
He has also sent me for an ultrasound scan to investigate the pain.
could the cyst possibly be a cause of the pain

Normally, epididymal cyst or spermatocele is painless. However, painful symptoms can occur. Your doctor should have checked for transillumination test which is negative in solid masses and positive in fluid filled cysts. The confirmation is ideally done via the ultrasound. Its a common and benign issue that is not related to malignancy or decreased fertility.

1.what will happen if the pain is caused by the cyst? Will this be shown on the ultrasound.

2.what would happen if it is not caused by the cyst? What other problems could it be? Would I need further tests and how would they be treated?

If the cyst is determined to be the cause of the pain then surgical removal can be an option. You will require a urologist to assess you for that. Ultrasound is an imaging investigation, it does not correlate the cause of the pain. That has to be done clinically.

As I mentioned before, an infection of the genitourinary tract is a likely cause especially with history of surgery along the same area.

Spermatocelectomy or Sclerotherapy can be offered for symptomatic cysts as treatment.

Hello.

A urologist would first examine you. Based on his clinical judgment and in light of the investigations ( urine test, blood counts, ultrasounds), he will make the diagnosis and may advise surgical removal of the cyst if present. He may advise antibiotics and NSAIDs for 2 weeks to see for empiric improvement. If the discomfort persists then surgical option can be resorted to.

The scan will be done by a doctor i.e a radiologist. It can be a male or a female. If at any point you feel discomfort , you may let your doctor know about it. But as far as the scan goes there should be no discomfort at all. You seem to be anxious about the scan. You should relax as its not an invasive investigation. Try to take deep and slow breaths while closing your eyes prior to the appointment to relax yourself. The manipulation or handling involved in this scan will not lead to erection or ejaculation. Your doctor will take you in to confidence and make you comfortable regarding the procedure.
