What Causes Terrible Headache While Masturbating?

Question: Hi dr. I have a question for you,
When I masturbate , I have a terrible headache in my head. I had it for the first time 4-5 days ago, I can'T wait it to dissapper.
When I was masturbating, it came slowly slowly but once I almost reach orgasm it hurt so much. Since, I have a lot of pain in my neck and in my had. :(

Good evening. I am an adult neurologist and headache specialist from the XXXXXXX OH area of the world. You are suffering from the not so uncommon headache type referred to orgasmic headache or coital headache. Your symptoms are typical of this sort of headache (for which nobody has a definite reason why or how it really comes about). All we know is that men suffer decidedly more frequently than women and it comes on really exactly how you described. In some cases the headache comes on explosively and without any warning just at the moment of climax. It can be so severe that it can literally cause people to "jump" off their partners and totally lose interest in sexual relations...even become scared of them.

There is a treatment for them though, however, it can be difficult for some folks to tolerate so it'll take some bit of testing on your part to see how it goes. But typically, Indomethacin is what is prescribed by first intention for this type of headache and should be taken about 60 min. prior to either intercourse or masturbation. Most significant side effects are GI in nature and can go from anything as mild as vague abdominal discomfort to XXXXXXX nausea, vomiting, terrible belly pain. The worst side effect that many people unfortunately end up with if on this medication for any length of time is blood in the stool. This typically represents the formation or exacerbation of a peptic ulcer of some sort.

There could be alternative medications but none with the efficacy for this type of headache.

I am going to provide you with a link that I want you to read that explains the headache you are describing.

In your case it slowly comes on and seems to last well beyond orgasm. Pain in the neck, back of the head, and even jaw muscle tightening may occur. Therefore, we would most likely refer to this as a PRE-ORGASMIC type of headache or HEADACHE secondary to SEXUAL ACTIVITY...in this case, masturbation.

This type of headache usually has no other cause associated with it except sexual activity. However, in some cases there is a cause that can be discovered but you need to see a specialist who can run the proper tests so that things such bleeding the brain, low cerebrospinal fluid pressure (spontaneous leaks), or arterial fistulae/other vascular anomalies can be ruled out. If a cause is found then, that would need to be addressed.

The headache usually goes away within 24 hrs. Many are much shorter in duration. It is important to recognize that these are not migraine headaches and should not be treated as such.

If these headaches only occur occasionally such as once weekly or less then, the medication INDOMETHACIN works about 80% of the time if taken about an hour or so before activities. If the headaches continue to come on and are more frequent than once a week then, prevention is best obtained by using PROPRANOLOL.

The proper dose has to be searched for in each person but usually somewhere between 40-60mg. daily (either as a once daily regimen or sometimes 2-3x/day) can prevent them from coming on. How long does this regimen have to last? Unfortunately, nobody knows why they come on in the first place. I usually recommend that my patients stay on the course of either treatment of the headaches if infrequent or prophylactic treatment for a minimum of 3-6 months before trying to retire the medication.
