Question: CAN YOU PROVIDE DIFFERENTIAL DIAGNOSIS IN 80 YEAR OLD MALE WITH TENDERNESS IN NIPPLES AND BREAST TISSUE BENEATH NIPPLES?

I have gone through your query.
It is difficult to comment on it without examination, I would like you to upload the pictures, I'll go through them, that will give me general idea what it might be.

I would like to know is there any history of injury?
Is there any redness or lump?
Is there any swelling?

Based on the information, I'll give you few possibilities, it might be due to crack in nipple which might be due to either due to external environment or he might have scratched it. Other possibility is infection of hair follicles around it.

Presently I would suggest you to apply warm compresses and he can take over the counter NSAIDS like ibuprofen or diclofenac tablet.
