What Causes Swollen Leg And Poor Urine Output After Kidney Transplantation?

Question: I have pain over my kidney transplant, my legs are swollen, and my urine output is decreased. My vital signs today are BP 169/92, HR 66 reg, RR 14, Temp 37.4 C, Weight 87 kg (2 weeks ago 85 kg), Ht 5'10", 3+ pitting edema in ankles, I know I must be rejecting the kidney allograft, but what lab values would tell you I am rejecting my transplant. What would cause hyperkalemia and leukoopenia,is there any other medical problems that you see that I should worry about. Any recommendations to treat hyperkalemia, luekipenia, and any recommendation for rejection medications.

Welcome to HCM and thank you for the query.
I understand your concern.
A high/ rising serum creatinine along with your symptoms is suggestive of a transplant rejection. Hyperkalemia is due to the inability of the kidneys to excrete the Potassium and is made worse by Potassium containing foods like fruits and fruit juices. Leukopenia is usually a side effect of the immunosuppressants or a viral infection. You should check with your treating doctor. You should see your doctor as early as possible and get yourself reassessed.
While you are waiting for the appointment you can reduce your fluid overload by decreasing the intake of salt and all fluids especially fruit juices. You may also use a diuretic like Frusemide if your doctor approves it.

Carbamazepine can decrease the level of tacrolimus which is an immunosuppressant protecting your transplant. If the neuropathy is not severe, it may be discontinued if your doctor agrees.
