What Causes Swollen Feet When Diagnosed With LVF?

Question: Hello Doctor, My mother is 76 years old and she has been diagnosed for Acute Inferior wall MI with LVEF of 30% with Acute LVF. she had an Acute renal failure from which she has recovered. She is now given the following Medication, Ecosprin, Clopilet, Atorva, Lasix, Aldactone, Doxobid, Isolazine, Lannoxin, Rablet-D. She is now detected for diabetes and her sugar level are in the range of 150-175. Please help me understand the reasons for using these many medicines and right now her feet are in swollen condition. Is there a need to change the tablet and if yes which one

Hello, Sir.

I can understand your concern.

Acute Inferior wall MI with LVEF of 30% with Acute LVF - implies that left ventricle of heart has failed significantly to function due to hear attack-MI.

The usual function is around 50-60% ,but in her case is 30%.

LVF(left ventricle failure) is characterized by fluid accumulation in the lungs. Additionally Inferior wall MI will affect Right ventricle that will cause fluid to accumulate in the feet initially giving swollen look.

Hence medications are prescribed to work at various levels to improve heart function.

ECOSPRIN and CLOPILET- given to prevent blood clotting and re-MI.

ATORVA- lowers the elevated blood lipid levels.

LASIX and ALDACTONE- will help to remove fluid from lungs and feet.

DOXOBID-Newer generation drug to prevent inflammation in the lungs.

ISOLAZINE-will help to reduce Blood pressure.

LANNOXIN- will help to improve the pumping action of a heart recovering from failure.

RABLET-D- will help to prevent acidity and gastritis.

As of now there is no need to change any drug.

In case if the feet swelling is worsening, it is important to monitor the LVEF again through 2D echo and escalate the dose of Lasix and Aldactone after consulting your cardiologist.

New onset diabetes need antidiabetic therapy soon. Please check with your diabetologist.
