Question: Hi. I woke up with swelling on both sides of my face in front of the ears a bit lower. It is slightly tender to the touch. What could this mean?!

Hi Madam,

I have carefully read your query and understand your concern.

The swelling in front of ears is usually from the parotid gland which is anatomically located in that specified area.

This is most likely to be infectious parotitis,most likely to be bacterial in origin in my opinion.

The infection usually ascends from the salivary gland ducts from the oral cavity.

I advice you to have a conservative management before proceeding to further investigations.

Tender parotitis responds to these measures in large number of cases.

A two week course of antibiotics like augmentin 625mg twice a day is all sufficient to have cleared infection and the swelling subsides.

Cold compress with twice a day antiseptic mouthwash like Listerine can have added benefits.

Note:these drugs have to be taken only under strict medical supervision.

I don't have any other symptoms of infection. No fever, no chills, aches, etc... No lymph nodes. If it is viral, Do you think it will go down if I use Ibuprofin for a few days?

welcome back,

You have mild inflammation of your parotids,hence systemic upset of fever and other features are not prominent or the pathology might be in early stage.

It could even be a viral cause,but the incidence at this age of yours is very low.

Ibuprofen an analgesic will certainly help to reduce the severity and duration of the symptoms.So you can take it under your doctor's prescription.

Antibiotics are recommended even in viral infections to prevent the secondary bacterial infections and reduce the mobidity.

Salivary gland infections usually respond to a combination of antibiotics like amoxicillin and clavulanic acid(augmentin).
