Brief Answer:
Possibility of congenital Hernia. Consult Pediatric Surgeon

Detailed Answer:
Hi.
Thanks for your query and an elucidate history; understood your concerns.
Swelling of one testicle- scrotum on one side in a child of 2 years which is not symptomatic and has st history is indicative of congenital hernia unless proved otherwise.
The tests are necessary to confirm or rule out any problem related to the urinary system.
I would advise you the following:
Consult a Pediatric Surgeon or a General Surgeon if Pediatric Surgeon is not available in your City for clinical evaluation, getting the correct diagnosis and proper treatment.

Brief Answer:
Specialist opinion and investigations and management.

Detailed Answer:
Hi.
Thanks for your feedback.
Since the child is only 2 years old, getting epididymo-orchitis is very very rare in this age group.
There may be a pre-existing lesion in the urethra or anything that is causing urinary tract infection.
I would still advise you to take him to the Pediatric Surgeon/ Urologist for further evaluation to find if there is any precipitating factor and get investigated and treated accordingly.
Also send urine for: Routine, microscopy, culture and sensitivity as well as tests of blood.
