What Causes Sudden Severe Rashes Around Pelvic Area In Child?

Question: We have twins (girls) and they are just about 7 months old. One of them has developed severe rashes around the pelvic area. This has been prominent for over a month now. We took our doctor's advice and have done all of the following:
- We keep her off diapers for most part of the day (completely off diapers at night)
- We make sure the area is dry and we apply the following creams regularly Nappy rash cream, Rash cream from Himalaya, Sabamed rash cream
- We give them bath daily and keep their beds dry all the time by using sheets that absorbs urine

After all these efforts the rashes are still prominent, can you recommend our next course of action? we are worried this has prolonged for so long.

She caught cold last week and due to which we kept her diaper on during nights for a couple of days and we saw that the rashes aggrevated

Thanks for asking on this platform

I can understand your concern.I would like to know
If the rashes are persistent for last one month or it improves and then recurs?
Is there any itching associated with rashes?
Are rashes limited to diaper region only or involving other parts too?

First thing I would suggest you to use diaper of another brand.
Sometimes baby may develop an allergy or intolerance to a particular diaper.
Sometimes persistent rashes which are difficult to manage can be due to candida dermatitis(prone to develop in diaper area due to moisture).
The clinical character of candida dermatitis is different from the usual diaper dermatitis(candida dermatitis has white satellite lesions)
I request you to attach an image of the rashes so that I better able to help you out.
Application of mild steroid cream helps in relieving persistent rash.
I would suggest you to apply SILODERM MIXI(ointment containing antifungal with mild steroid) cream for 5-7 days for better effect.

Thanks for your response doctor, here are the answers to your question:

If the rashes are persistent for last one month or it improves and then recurs?
ans: The rashes improves slightly and recurs
Is there any itching associated with rashes?
ans: not that we have noticed, but we are guessing she has burning sensation. we've heard her crying sometimes when she feels uncomfortable.
Are rashes limited to diaper region only or involving other parts too?
Ans: its limited to diaper region only

We have tried diapers of different brands but they are all of the same effect.

I'll try to get a pic of the affected area shortly.

Please advice if I should proceed with applying SILODERM?

Hello Doctor,

As discussed I have attached pics of the affected area, I also want to update that the same leison was observed in the neck region and a mildly in the underarms a week before. For the neck region we applied a combination of B4 nappi creame and zole ointment, its partially healed but that area has turned black.
Could you please review the pics and let me know if you want us to proceed with applying SILODERM MIXI for both neck and the pelvic area?

I have gone through the images and it appears to be candida dermatitis.

As I mentioned earlier you can see white satellite lesions which is suggestive of fungal infection.
You have mentioned that there is improvement after application of zole (miconazole an antifungal) ointment.This also indicates that there is fungal infection.

There are blackened area in the neck.I would suggest you to apply SILODERM MIXI cream in both the area as it also contains steroid which helps in early recovery and will also improve the skin color.
Apart from this take all those measures like frequent change of nappy,to keep the area dry and avoid moisture.

I shall be glad to answer if you have any more query.
