Posted on Fri, 19 Feb 2016
Question: Please can somebody help me as this is making me so miserable :( I suddenly became unwell in XXXXXXX 2015, lump sensation in throat, nausea and unable to eat causing my BMI to go to 16.4, I had several tests, bloods, chest scan and endoscopy and all test results came back ok and after time things improved slowly and I was able to have some good days but now I'm starting to feel unwell again.
I feel like there is phelm coming up my oesophagus daily like I want to cough or be sick, I also have to clear my throat, sometimes it's so bad I am unable to eat, my chest is so tight sometimes and even achy but not all the time and if I have a hot shower I feel like I can hardly breath. I never have a cough, I do not get pain and I have never been sick with it. I do have to take anti histamine tablets daily otherwise my chest is worse along with itchy eyes and nose.
I am a 33 year old women with no existing health problems, I do not smoke and my BMI is currently at 18.

Brief Answer:
? GERD could be the cause for your symptoms, need to relate clinically

Detailed Answer:
Hello ma'am and welcome.

Thank you for writing to us.

I have gone through your query with diligence and would like you to know that I am here to help. I would like to ask if you have undergone a test called a '24-hour acid pH monitoring'? If not, then I would highly recommend it.

Until you can get that done, here is my list of suggestions:

+Avoid acidic food
+Avoid oily food
+Avoid fatty food
+Avoid fried food
+Avoid spicy food
+Avoid smoking
+Avoid alcohol
+Avoid carbonated drinks
+Avoid stress
+Drink plenty of water
+Have 5-6 smaller meals instead of 2-3 larger ones
+Avoid overeating
+Avoid long gaps between meals
+Lose weight if you are overweight
+Have 1 tablet of 30 mg lansoprazole once every morning 15 minutes before breakfast
+You also have 1 tablet of ranitidine or omeprazole 15 minutes before any meal in case you have any symptoms presenting

I would also like to ask you a few more questions, and would appreciate your response on them all, thank you:

1. How many of the above listed suggestions of mine would imply to you?
2. Can you associate your symptoms worsening or appearing being related to your diet (e.g. alcohol consumption, festive food, overeating, stress, etc.)?
3. How many of the following symptoms do you have:

-Dry cough
-Sensation of a lump at the back of your throat
-Regurgitation of food or liquid into the mouth
-Bitter taste in the mouth
-Heartburn
-Sore throat
-Difficulty swallowing
-Hoarseness of voice

I look forward to hearing from you in order to help you better.

Best wishes.

Thank you for your quick reply, no I have not had a 24 hour acid ph test, how do I go about having one of these?

I was under stress when the symptoms first started in XXXXXXX however I am not under any stress now.

In relation to your questions, approximately 7 things in your list I can relate to, I'm also on no medication at the moment so it would be good to try lansoprazole, I will have to ask my doctor for them.

I can put a link to stress to it and I also can't stomach fried or acid foods anymore as if I do have them I spend hours feeling sick afterwards.

As for symptoms I do get a lump sensation in my throat, I do have swallowing problems, sore throat, occasional heartburn and a hoarse voice.

Brief Answer:
GERD is the most likely culprit

Detailed Answer:
Hello once again ma'am.

Your diagnosis could quite possibly be a condition called gastroesophageal reflux disease (GERD), you can read more about this or speak to your doctor about it. But I have either way mentioned my advise with the '+' sign, so please do follow that and you shall surely experience an improvement if you do so religiously.

Everything in your last response points towards GERD, and so I strongly suspect that to be the cause to your symptoms.

As for the 24-hour acid pH monitoring, you will have to speak to your doctor about it, but it shouldn't be too difficult to find in all major hospitals and clinics.

I hope you find my response helpful. Please do not hesitate to write to me for any further clarifications, I am always here to help.

Best wishes.
