What Causes Sudden Lightheadedness And Reduced Heart Rate?

Question: Hello,
I am a 31 year old male with no medical history. I had a medical check up about 3 months ago and this was normal ekg ecg blood work stress test all showed good health and strong fitness. Recently i had a few glasses of beer the weather was very hot and then i had some tea. I fell asleep woke up and climbed the stairs to go and use the bathroom. As i started to urinate i began to feel quite lightheaded as though as i was about to faint and also felt the heart rate slowing down i guess relating to bp. My question is what could this have been it has not happened again and this was the first time that it happened. With all my normal heart checks could something have changed in 3 months to cause this? I do regular exercise eat well and weigh about 64 kg. please advise

I have carefully gone through and analyzed the details provided. The phenomenon you mention, is called micturation syncope. It so happens that sometimes when you try to pass urine (micturate), the blood flow rushes to the bladder muscles as you concentrate on contracting the muscles while voiding. But in the process, the blood flow to the brain transiently comes down. Since the brain does not have any glucose or oxygen reserves, it is not unusual to feel lightheaded or even faint in such a condition. I would not consider this to be something to worry about and no investigation will reveal anything. Just continue a healthy lifestyle along with regular exercise and you would be fine.

Hello Doctor,
Thanks for providing some information here? Two questions does the fact i had been drinking and had just woken up contributed to this happening as i have not experienced this before? Also given that i had all the tests 3 months ago and everything was fine, could anything have changed from a cardiac point of view in this small time? Is micturition sycope ever caused by cardiac reasons?

Hi,
Just a couple more follow ups am just wondering what caused it to occur as it has not happened before even when I have had some alcohol is it because I just got up from sleeping and then climbed the stairs?
Also can you explain what you mean by the cause of syncope is only indirectly related to cardiac causes?

Thanks again for writing back. It could well be triggered due to the combined effect of alcohol on going to pee just getting up from sleep and climbing stairs.
Syncope is a phenomenon related to the cardiovascular system which is connected to the pumping of the blood to various parts of the body and the failure to pump blood adequately to the brain, even temporarily, is actually related to the cardiovascular system (or cardiac system). This is what I meant by indirectly related. Directly, your heart is not in a compromised state and you need not worry about it.

Hello Dr,
I have a couple of follow ups. What makes you say that the heart is not in a compromised state? I am just trying to establish how one can be sure this type of thing is not related to the heart.

Based on my clinical experience and understanding of pathophysiology it seems to me that it is not due to a compromised state of the heart. However, for peace of mind you can get a echocardiography done and see for yourself. It will establish the functional capacity of your heart.

But I just had an echo done only 3 months ago and it was fine a stress test was fine and so was an EKG and blood tests. The blood tests were all normal. So would I need to do tests again only 3 months later? I mean you mentioned that not much would change in 3 months. Or can that not be the case?

I clearly understand that the micturiton syncope is not likely to be indicative of any compromise in terms of the heart. I have already expressed that, but still you seem apprehensive. The echocardiography done 3 months back shows that your heart is fine, but still your apprehension does not come down. So is not not better to get a repeat echocardiogram done to demonstrate the present condition of the heart? I myself do not have this apprehension and would not otherwise have requested for this test.
