Question: My name is XXXXXXX and I am a forty eight year old in pretty good health female. The last five days or so I have been noticing that my neck and surrounding muscle area has been sore. And I touched the middle of my throat and it seemed a little sore. My ears have also been itching more lately in the past few days. However, today after eating a few bites of carrots and hummus and attempting to eat half a sandwich with lots of lettuce, I suddenly noticed instantly it seemed hard to swallow as if something is in my throat when I swallow. This completely alarmed me and then sensation is still ongoing( 3 hrs up till now). The right side of my throat feels full where my ear is a little itchy and everytime I swallow I get that sensations as something is in my throat. This past weekend I went on a small vacation and stayed at a hotel where I was bit pretty bad by bed bugs. The bites became infected! They were large and raised and very itchy and bothersome up until today. I treated the bites with an over the counter anti itch cream and sprayed bactine on the bites. That seemed to work. I tried to eat agan and the same sensations happen it felt as if something is in my throat and it feels inflamed and very bothersome. It has calmed down some what now but my esophagus still feels inflamed as if sore deep inside of it. I took 2 advils at 2:30 PST. Can you please tell me what could be possibly the issue. Thank you.

Brief Answer:
it could be anaphylactic reaction

Detailed Answer:
Hello,

I am sorry about the whole issue. You seem to have developed an anaphylactic reaction to the food that you consumed. In addition, the bite from the bed bugs made it worse.

Now, coming to the treatment - You could take antihistamines like Zyrtec 10mg at bed time one tablet for 7 days minimum. This will reduce the symptoms and help to prevent further reactions, if any.

Secondly, you could take Capsule Omeprazole 20mg on empty stomach early in the morning for a minimum of 7 days again. This will take care of the oesophageal irritation and inflammation, if any.

Both the medicines are available over the counter for you.

In addition, you can apply calamine lotion on the affected areas of skin to soothen them and help in faster healing. Apply it on the neck and the surrounding muscle area where you feel sore as well.

Hope this helps you and you get well soon.

Thank you doctor. Can I take both these medicines at the same time of course one for day and the other for night? I seemed to have slept pretty well with little irritation but now it seems to be starting up again, this morning now that I am up? Can you tell me why I am feeling the sensations again in my throat and this wasnt happening while sleep nor was I awakened by it? Thank you again for all your help.
I also forgot to mention that the night before yesterday I suddenly felt achy as if I was coming down with a cold. This went on for several hours until I took some Advil to help relieve the symptoms. I am guessing I must have been allergic to the bed bugs. This is so upsetting to me. I am feeling this tightness in my throat again this morning. Thank you. I am going to purchase the medications this morning to suggested. Also, are you allowed to prescribe medications?

Brief Answer:
all specific answers are below

Detailed Answer:
Hi XXXXXXX

Yes, you can take both these medicines at the same time. However, Omeprazole has to be taken on empty stomach while Zyrtec has to be taken after food. You can take at the same time, no harm in it.

There is residual anaphylactic reaction that is left over, that is the reason for the sensations coming back again. With Zyrtec tablet it will stop.

We are allowed to mention over the counter available drugs. However, we cannot prescribe prescription drugs. That would be medically unethical.

Hope you have gotten the answers.

Hello Doctor,
I followed your instructions and got the medications your prescribed. Thank you! I am feeling much better now. I talked with the physician pharmacist and she was helpful in helping me find the medications your prescribed. She also included that I should seek getting an epi pen in case an allergic reaction occurs again. I do realize that I need a prescription for that. I do not have medical insurance unfortunately at this time, but I have the option to visit a doctor and pay cash. Would you suggest I take a copy of our communication as evidence that I am having an allergic reaction so I can get this epi pen? Please advise. And again thank you so much!!

Brief Answer:
yes, please take a copy of our communication

Detailed Answer:
Hello XXXXXXX

Yes, please do take a copy of our communication so that it becomes easier for the doctor to arrive at a conclusion.

Also, yes, you can take the Epi pen shot if symptoms do not improve. I suppose your doctor should give you that without any hesitation.

Get well soon.
