Local inflammatory skin conditions, can be a possibility if it's painful special of pain is coming out from the inside or in the bones or joints, a Xray if hand may help to rule out any tophus of gout inside.

Though one or two such lesions are insignificant but if it involves more fingers, workup with uric acid levels and detailed evaluation for autoimmune conditions or vasculitis profile may be needed after a carefully examination of hand by your primary doctor.
