Brief Answer:
Stress probably

Detailed Answer:
Hello,

if you've done such an extensive testing and you no:
- blood pressure problems (orthostatic hypotension or arterial hypertension)
- electrolyte disorders
- thyroid problems
- cardiac problems (normal echo, ECG)
- lung problems (normal chest X-ray and pulmonary function tests-spirometry)
then I believe you can safely attribute such symptoms to anxiety and psychological stress.
Notice the improvement when your mind is distracted like when exercising or less tense like when drinking alcohol.
Perhaps you should try some anxiolytics or beta blockers under your doctor's supervision.

I hope I've helped!
If you'd like more information, please let me know.

Kind Regards!
