What Causes Severe Upper Abdominal Pain During Bowel Movement?

Question: I am female and 51. overweight with sleep apnea which is being treated. in great health otherwise. my labs are perfect great blood pressure and I exercise regularly. I am regular with bowels as well. here is my problem. My stomach frequently feels like I have taken medicine without food. I have not. Once in awhile I get extreme upper abdominal pain while having a bowel movement. it is on the right side between my hip bone and my rib cage. it is a horrible tearing feeling. There is no discoloration of the stools and no blood. The pain does not linger. The only medications I take are OTC allergy pills and Nexium 24 hour.

Dear patient,

if you were my patient, i would rule out gall bladder issues like gall stones through ultrasound of the abdomen first. If that is not it i would schedule a gastroscopy to rule out any gastritis or ulcer.
Spcific treatment like gall bladder surgery or antacids would follow depending on the results.
