What Causes Severe Shoulder Pain?
Question: Right shoulder pain throbbing in nature since 20 days. Insidious in onset and progressive in nature. Pain is aggravated on doing work and on applying pressure voluntarily and there are no relieving factors. Pain started after doing extremely strenuous household work 20 days back. No associated complaints with the pain.
ESR : 21 mm; S. uric acid : 3.7 mg/dl; RBS : 106 mg/dl. Took MICROCID-SR twice a day for 4.5 days that is 9 tablets.

As per your age and history there is possibility of shoulder arthritis.
Since ESR is raised suggest long term ongoing disease process.
Another possibility of early shoulder pericapsulitis can be considered.
You need clinical examination from orthopaedic surgeon to know the origin of pain and investigation accordingly.
Investigation like MRI cervical spine/shoulder, EMG-NCV [Nerve conduction study], CPK and x ray shoulder with humerus may help.
I will advise to check your vit B12 and vit D3 level.
You can consult physiotherapist for help.
Physiotherapy like ultrasound and interferential therapy will give quick relief
Strengthening gradual exercises will help in reducing pain.
