Question: i had been taking monopril 40mg/day for about 8-10 yrs without major side effects, some minor. i was recently started on adactone 25mg daily about 3 months ago . in the past 3-4 weeks i developed severe pruritis, then a rash on my neck, then a burning and redness of the skin like a bad sunburn, then a severe red rash on face neck and chest, now some spots of rash in groin area.i discontinued aldactone 2 weeks ago. i having continued taking monopril but in small dosages, like a quater tab at a time. i also have burning and tingling of tongue, mouth and lips. i have been using hydrocortisone cream, eucerin lotion and benadryl which has helped some. my doctor has also ordered Amlodpine 2.5 mg to replace the aldactone, but i am recluctant to add another med in the mix when i'm continuing with these sxs. 1.do you think i am having an allegic reaction to the fosinpril as well as the aldactone? do you think i should add the amlodipine and /or stop the fosinopril

Detailed Answer:
Hello!

Welcome and thank you for asking on HCM!

Your symptoms seem to be related to an adverse effect of adactone. You should know that allergic reactions are a common adverse effect of this drug.

But, as you have been taking monopril for years, I don't think that you should stop it, because it is not likely that your complaints are related to this drug.

From the other hand the tingling in the tongue, lips and mouth could be a symptoms of hyperkalemia (high potassium levels). Both monopril and adactone can increase potassium levels.

That is why I would recommend checking your potassium plasma levels.

Regarding amlodipine, I agree with your doctor decision on starting this drug, as it is not known to cause allergic reactions. It will help manage better your blood pressure. Besides, it does not increase plasma potassium levels. I would just recommend starting in low doses in order to adjust the dose based on your blood pressure values and avoid possible hypotension (low blood pressure).

Hope to have been helpful!

Wishing good health,

Dr. Iliri
