What Causes Severe Pain In Testicles?

Question: Hello I am 18 and last night I was laying on my side and when I got up I had severe pain in my spermatic cord and testicles. My testicle was slightly swollen and if I pressed again the top of my spermatic cord before I get to my lower abdomen it causes severe pain. I went to the gym Tuesday night and did some moderate lifting but nothing that shouldn't have hurt me. My testicle seemed to turn vertically abnormally on and off. Now it is morning after and I have very little feeling in my testicle but still pain in my spermatic cord if I move around some and slight testicle pain depending on where I touch it. My testicle has now shrunken to
an abnormal size smaller than my left. My left testicle on a normal basis hangs lower. The pain is still here along the right testicle and cords. I thought it would go away overnight. I was very afraid it was testicular torsion if it is it's likely to late which I'm afraid of and I have no transportation to getting there. My testicle still does abnormal rising and lowering while the left one is at a normal state as I type. Please I am very afraid what do you think it is? It'll likely be another week before I be able to visit my doctor. If it's torsion I'm pretty much guaranteed by then 1 testicle which I am afraid of. I have never had sex but I do masturbate daily but not in the past 2 days.Thank you in advance.

Thanks for your query and an elucidate history, albeit it is a bit confusing the way you have put it.

One thing is for sure that you are suffering from recurrent testicular torsion due to anatomical variation or a strong cremaster response or loosely hanging testicle like a clapper in bell appearance allowing the abnormal vertical turning or abnormal rising and lowering as you have described.

The severe pain can be due to torsion or a retrograde infection of the cord, testis and epididymis - Epididymo-orchitis the inflammation spreading the to spermatic cord too.

What wonders me is that you have mentioned you had Gym a few days back and you got the symptoms after. In the morning you could get very little feeling in the testicle but pain in the cord, now the testicle is shrunken smaller than the left one. It appears that it light have been smaller than the left since some time ago. The left testis is always lower than the right in most of the males as a natural phenomenon.

The status of the testis can be decided only by a physical examination preferably done by an Urologist /Surgeon and ultrasonography with color Doppler examination. You also need to undergo few other tests:
Urine: routine, microscopy, culture and sensitivity.
Blood: CBC, sugar, urea, creatinine.
Semen analysis as a baseline.
Ultrasonography and color Doppler as already suggested.

Once the sample for the tests have been given you have to get started on appropriate antibiotics and supportive medicines.
Give nice scrotal support, drink plenty of water and fluids orally, take full rest, no strenuous activity at all, take a mild laxative as straining at stool also can enhance torsion, visit the specialist as soon as possible.

Thank you doctor. I know that torsion has a time limit. this started around 12am last night it is now 3 on today... Do I still have time to save my testicle?
